Rheumatoid Arthritis Fixing The Feet
To see Audrey Walston move today, you'd never guess how far she's come. "It was painful to walk, to move, to put a shoe on. It got that I couldn't wear shoes because the foot had got quite deformed," says Walston. Audrey has rheumatoid arthritis. An autoimmune disease, it attacks the joints throughout the body. "And by doing that, it creates a lot of swelling, inflammation, pain. And that creates deformity if it left unchecked," explains Dr. Andrew Belis. He is a foot and ankle surgeon on medical staff of Lee Memorial Health System. That's exactly what happened to Audrey- her hands and feet were especially hard hit. "It can affect the feet where the toes and the joints of the feet collapse or dislocate or angulate, usually in an outward appearance," says Dr. Belis. Her X-rays reveal Audrey's feel were so damaged she could barely function. The pain was unbearable. "I was ready to chop a leg off if that would've taken some of the pain away or improved my quality of life," says Walston. That's when she made her way to Dr. Belis. "She was in bad shape. She came in with a kind of 'walking on the ankles' and really severe inverted, flat-foot type problem." By the time rheumatoid arthritis patients seek out surgery, they've usually been on medications for many years. Being a progressive disease, the joints are damaged over time. Doctors may use several surgical techniques from tendon lengthening to joint fusion to rebuild the foot from the ground up. "We did some minimally invasive things up near her toes because the joints were not too, too far gone. In the end we did multiple fusions and we used internal hardware to hold it in place. And she did very well," says Dr. Belis. Her feet look much better, but it's nothing compared to how she feels. "I figured I won the lotto, getting around the way I do," Walston exclaims. In many cases, exercising your foot can help reduce the pain and promote toe flexibility. It can also slow down the progression of your bunion, thus preventing the need for surgery. Daily exercise also helps strengthen the muscles of the legs and feet and can prevent bunions from developing. Exercise 1: Ball Rolling Place your foot on a tennis or golf ball and roll the ball beneath the mounds of your toes. Then roll on anywhere on sole that feels tight. Exercise 2: Interlace Fingers Between Toes You need to do this by sitting, then cup the sole of the affected foot in your palm. With the free hand available, interlace the fingers between the toes of your foot. Then begin by bending the toes back and forth, upward and downward for about 5 to 10 minutes. Exercise 3: Curl Toes Do this exercise by stand up straight or sitting. Then point your toes straight ahead for 10 seconds, and then curl them under for 10 seconds. Repeat this for several times. Exercise 4: Band Stretching Remember to use a good elastic band for this exercise. Place them around the big toes on each foot. Then slowly turn your feet outward until you feel the stretch on the big toes. Hold the position for 1 minute and then relax. Perform this exercise for 15 minutes for 2 or 3 times a day. These are some exercises that you can easily perform at home and anyplace. If you have Bunions and recently causing sore and inflamed after you have been on your feet for too long. Relieve pain and inflammation using ice therapy is a great method because cold temperature temporarily numbs the nerve endings and provides relief pain. 1. Place a handful of crushed ice inside thin towel and wrap it. 2. Begin to apply on affected area for 5 to 10 minutes. 3. Then break for 10 minutes and repeat the procedure 2 or 3 more times. 4. Use this simple remedy as needed. In order to reduce your chances of growing bunion also to prevent from need of surgery if you already have one, take note focusing your footwear. Always wear footwear with wide front, broad toes and soft soles. Also use padded shoe inserts to support distribution of pressure evenly when you move your feet. If you have a bunion, wear or choose shoes with a wide and deep toe area. Sandals are a good choice. Do not wear shoes with heels higher than 2.25 inches. Prevent from wearing high-heeled shoes for longer than 3 hours at a time. Choosing footwear that fit the shape of your feet is best. Never force your foot into a shoe that does not fit while also avoid tight fitting socks or stockings. An autoimmune disease, rheumatoid arthritis wreaks havoc on the body's joints. "Your body destroys or reacts with the joint tissue and the synovial lining of the joint. And by doing that it creates a lot of swelling, inflammation, pain. When it comes to the feet, rheumatoid arthritis is an arch nemesis. "The flatter foot type, the collapse of the joints, deviation of the toes, hammering of the toes or bunions and hallux valgus problems," says Dr. Belis. The next step in many cases is that patients have trouble walking. A foot surgeon can use several techniques to correct deviations, restore balance and lessen pain. "If it's just a contracted joint that's just in the early stages sometimes just doing some simple lengthening of the tendons can help strengthen them out. If it's caused a little more joint degeneration of the joint surface itself, then sometimes the joints are not salvageable at all," says Dr. Belis. In those cases a fusion might restore the most function. Before it gets to that point, a specialist may intervene with some step savers. "We definitely want to support the foot's structure. DR Nabil Ebraheim UTMC Toledo. The cartilage of the knee is complex and it is made of elastic comprehensive structure. The normal articular cartilage is called hyaline cartilage; it provides a smooth, gliding surface to help the motion of the joint. There is about 2cc of the synovial fluid inside the knee that helps in the motion and lubrication of the joint. Between the hyaline cartilage which is called the articular cartilage you can see the meniscus, the lateral and the medial, the meniscus is a shock absorbing cartilage or cushion between the articular cartilages. The hyaline cartilage has 4 layers: 1- The superficial layer. 2- The middle layer. 3- The deep layer. 4- The calcified layer After the calcified layer you find the bone. These cartilage cells are supposed to live forever. Good cartilage cells are sterile; they can’t make more cartilage if these cartilage cells are destroyed. If the cartilage is subjected to excessive wear, trauma, injury, overuse, excessive weight or improper alignment, then the cartilage will wear away leaving the bone to rub against bone. The cartilage doesn’t have the ability to heal itself by hyaline cartilage, but sometimes it can heal itself by inferior type of cartilage called fibrocartilage, especially if the area that needs to be repaired is small. What is the treatment of the arthritis of the knee? 1- Medication: people respond differently to medications, the doctor select the type, dosage and duration of the treatment, the medication has to be safe and effective. Ask the doctor about the side effect of the medication. There are several medications for example: NSAIDS. 2- Losing Weight: it’s advisable to lose about 5% of the body weight, especially if the BMI was above 25. 3- Low Impact Physical Therapy: such activities like swimming or cycling, adds less stress on the knees, life style modification to protect the knee will slow the progress of the arthritis, there is a strong evidence that physical therapy will help the patient. The physiotherapy will decrease the pain, will improve the function, increase the strength, the range of motion; physiotherapy should be individualized with program that meets the patient’s needs, the life style, and expectations. 4- The physician will also use intra-articular injection of steroids; viscosupplementation, or hyaluronic acid, injection will relieve the patient’s pain and disability. 5- Other injection methods: - Gene therapy - PRP - Growth factor - Stem cells 6- Other treatment options: - Acupuncture - Massage - Glucosamine - Chondroitin sulfate - Valgus directing brace - Wedges in the foot - Arthroscopic debridement and lavage 7- Assistance devices: - Cane - Shock absorbing shoe - Shoe inserts - Knee sleeves - Support brace 8- Arthroscopy shouldn’t be done in arthritis unless there is loose body or meniscal tear causing mechanical symptoms of recurring lacking, catching, swelling and pain. There are some guide lines from the American Academy of Orthopaedic Surgeon AAOS: these guidelines are suggestive, and the treatment of the arthritis patient should be individualized and based on: - The doctor’s clinical judgment. - The patient’s clinical situation. - The evidence tested and published. It is a combination of all of these factors. If it works for you, it may not work for your neighbor. So basically arthritis can be minimal, and anything can help the arthritis ion this situation. But if the arthritis is moderate then the physician has many options for treatment and none of those options are predictable in the result. When the arthritis is severe, total knee is the most predictable option for treatment of arthritis. You have to reduce the pain and improve the function of the knee. Don’t Mix Up Arthritis with Plantar Fasciitis
The very painful foot condition called plantar fasciitis affects your life with every step you take. The major symptom of plantar fasciitis is a pain in your foot. It is usually localized at the bottom of your foot (the sole) but the pain may feel like it’s radiating throughout parts of your foot, ankle, and lower leg. Because of this, plantar fasciitis may be confused or mistaken with other conditions affecting your foot. Many other maladies may cause foot pain and can be mistaken for plantar fasciitis, so don’t mix them up! The main one is arthritis, a common problem that many people suffer from somewhere in their body. When arthritis occurs in the lower leg, ankle, or some part of the foot it can cause a pain that may be felt the same way as pain from plantar fasciitis. A primary difference is that arthritis pain is usually worse when the joint is being used. When the joint is at rest then the pain may not even occur, which is the same pattern you see in plantar fasciitis. So you may have arthritis in your heel and only notice it when you take a step and engage the heel. Also, with arteritis it takes longer (if at all) to warm up your foot and ease the pain. Arthritis can be diagnosed with a more thorough workup by your doctor. In order to prevent plantar fasciitis morning heel pain, simply wear a night brace. This keeps your ankle in a neutral position (or right angle) while you sleep and alleviates the morning pain.
