E/M with Diabetes | E&M Coding Tips http://www.cco.us/product/em-ceu-credits-online-course/ Q: E/M with Diabetes - “When coding say an E/M encounter and the patient has type 2 diabetes documented in the medical record but the patient is not seeing the provider for her diabetes, is it still listed as a diagnosis?” A: This one I happen to know as a risk adjustment point of view. I would tell you any time that a patient comes in that they have a chronic condition especially diabetes that affects all the other body systems, you definitely are going to have it listed as a diagnosis. One of the things that the providers are struggling with these past few years is because they don’t do this. Let’s say we have a scenario which is really hard for us to find a scenario where diabetes isn’t pertinent. If a person comes in with, just say, a respiratory infection and it’s not that big of a deal but he prescribes antibiotic and let’s say he gives him a little bit of a steroid, it’s starting to get into the lungs. He may not list that the person is a diabetic; however, I look at it as a point of view, if he give a diabetic any type of steroid, it’s going to change their blood sugars. List of drugs that raise blood sugar. This information can come in handy in case you have high blood sugar and you are not sure what is causing it. Download Diabetes Diet eBook: https://goo.gl/UC5SZU We all get sick from time to time. We all have been given prescriptions other than our metformin or insulin. But did you know some medications can raise your blood sugar? I am confident that somewhere in that plethora of literature that accompanies most prescriptions is information that says that the drug will affect blood sugar. But how many of us actually read all that literature? Today I want to talk about common drugs that can raise your blood sugar. This list was compiled after researching the work of Mark T. Marino, MD. The list: Corticosteroids Thiazide diuretics Beta-blockers Niacin Antipsychotic drugs Striking a balance There is a longer list of drugs associated with diabetes, but the evidence for each of these is not very strong, or the medicine is only used rarely. For the more commonly used drugs that are known to worsen diabetes control, it is important to note that there remains a balance between intended benefits and unwanted side effects. Glucocorticoids can stop an inflammatory process like asthma, thiazide diuretics have been shown to be very successful at reducing high blood pressure and its complications, beta-blockers do protect against heart attacks, niacin does reduce coronary artery disease, and antipsychotic medicines can make the difference between hallucinating and perceiving reality. If you suspect that your blood glucose control may be adversely affected by a medicine you’re taking, speak with your health-care provider before making any changes. There may be an overall benefit to remaining on a drug that slightly increases blood glucose in some cases, or there may be alternatives that can be substituted. Unfortunately, no medicine is without its drawbacks, but by knowing what these drawbacks are, you can participate in an informed discussion with your health-care provider about the specific risks and benefits of a drug in your unique situation.

These recent studies may lead to better understanding and ultimate treatment of the disease, when both low testosterone and diabetes are present in men. Prednisone is a pretty complicated drug that many people of all ages may have to take at some point. Some of us - like myself - have been on it chronically, on and off, for many years. I wanted to use my experience and the experience of hundreds of others I've talked to who have been on Prednisone to offer some tips on how to deal with Prednisone. Knowledge about this drug and it's side effects can make it much easier to deal with and that's the goal! I hope you all find some info you can use and please feel free to message or comment any questions you have and I'll be sure to answer them:) **Disclaimer** I'm not a doctor just someone who has dealt with Prednsone over the last 20 years so unless something is super obvious, please talk to your doctor before implementing any of these techniques! If a person has high blood sugar, it is probably due to diabetes. Diabetes mellitus generally refers to the bodys inability to produce insulin that controls the release and absorption of sugar or glucose in the blood stream. Symptoms of Diabetes : The typical symptoms of high blood sugar are frequent urination, increased thirst and increased hunger. In most types of diabetes, the symptoms are likely to be similar to the symptoms of high blood sugar. The primary symptoms of Type 1 diabetes are nausea and vomiting. In type 1 diabetes, symptoms generally develop rather quickly. Symptoms of type 2 diabetes are often confused with symptoms of obesity or ageing. Such symptoms include irritability, itchy skin, yeast infections, dry mouth and leg pain. Causes of Diabetes Mellitus: Type 1 diabetes is generally caused due to auto immune changes in the body. In type 1 diabetes, the body attacks cells in the pancreas that produce insulin. Type 1 diabetes is generally more prevalent in men as compared to women. Type 2 diabetes is mostly caused by genetic defects.Steroids and stress are generally known to be causes of Type 2 diabetes other than inherited genetic defects. Treatment : 01.Bitter Gourd Bitter gourd, also known as bitter melon, can be helpful for controlling diabetes due to its blood glucose lowering effects. It tends to influence the glucose metabolism all over your body rather than a particular organ or tissue. • Drink some bitter gourd juice on an empty stomach each morning. First remove the seeds of two to three bitter gourds and use a juicer to extract the juice. Add some water and then drink it. Follow this treatment daily in the morning for at least two months • Also, you can include one dish made of bitter gourd daily in your diet. 02. Mango Leaves: The delicate and tender mango leaves can be used to treat diabetes by regulating insulin levels in the blood. There are a few structures in the body that we call Bursa. Bursa are little sacs with a tiny little bit of fluid in them that are designed to allow tendons and muscles to slide over each other. Two very common ones that run into trouble: are ones in the shoulder, or the ones in the hip. And when they run into trouble, we call it Bursitis. But sometimes, that’s not actually the most accurate title for it. Bursitis implies that there’s inflammation because –itis, at the end of every word means inflammation. Sometimes it’s a problem of shortened muscles putting too much pressure on the structures in or through the Bursa. And sometimes in the hip in particular, it’s tightened certain muscles that are too tight and pressed on the Bursa. So it’s a matter of working with a chiropractor or a sports physician or a physical therapist or trainer or massage therapist who understands how to recognize what all the other muscles around the Bursa are doing and really recognizing that the Bursa can be just an innocent bystander. Sometimes people do get relief from injections of steroid into the Bursa but sometimes they don’t. If you’ve had relief from the first one, that’s good but then you need to find out what the root cause of the problem is because it isn’t acceptable to keep having steroid injections. Injections of cortisone actually breakdown our tissues. It can breakdown muscle and tendon and it’s not very good for our bones either. It can make people who are diabetic or pre-diabetic have trouble controlling their blood sugar, that’s not very good for them either. And for some people it can also make them feel very stressed and agitated because cortisone is related to our stress hormones and some people react psychologically very badly to them. So I don’t recommend steroid injections unless we’re absolutely certain that that’s the only thing that will help. I think having a physical response like acupuncture for example, would be a good first step. ------------------------------------------ MEET DR. The emphasis is understanding these hormones and ways to promote them naturally. Please like, subscribe, comment and share! SUBSCRIBE: https://www.youtube.com/user/DrJohnKiel FACEBOOK: https://www.facebook.com/medicinelifestyle TWITTER: https://twitter.com/LifestyleMedYou I wanted to take a few minutes to explain Insulin-Like Growth Factor 1 to you. This molecule is also called somatostatin C and more often IGF-1 for short. This hormone is widely discussed in the sports world for possible performance enhancement. It also shares the common name insulin with the medication used to treat type 1 diabetes. For these reasons, I thought it would be worth discussing and spending some time reviewing. Briefly, I will discuss the anatomy, physiology and regulation of IGF-1. IGF-1 secretion is regulated by the release of human growth hormone, which is secreted from the pituitary gland. I have made a detailed video explaining growth hormone and I would encourage you to check that out if you want to know more. The function of IGF-1 and HGH overlap significantly and the distinction between the effects of HGH and IGF-1 are not entirely clear, and the research in this area is ongoing. IGF-1 is produced and secreted primarily from the liver, although other peripheral tissues, such as bone, produces its own IGF-1. The reason that insulin-like growth factor has the name is that it has an insulin like subunit that binds to insulin receptors, but only has 10% of the potency of regular insulin on glucose regulation. This is an anabolic hormone; in other words it ‘promotes growth’. While this includes skeletal muscle, it’s certainly not limited to it. It’s worth noting that it is not androgenic, as opposed to testosterone, meaning it doesn’t promote male sex characteristics such as facial hair and deeper voice. It’s primary effect is growth stimulation. This is a balanced effect which affects all tissues equally, not just skeletal muscle. This occurs because it is released into the bloodstream by the liver, although local production by bones probably is a little more effective locally. Production peaks during puberty where whole body growth is at it’s greatest. Other functions: Whole body protein synthesis and inhibition of protein breakdown (proteolysis) Enhanced bone mineralization Low blood sugar (hypoglycemia, remember it acts like insulin) Positive nitrogen balance Increased kidney filtration Partial reversal of catabolic activity of glucocorticoids on protein synthesis Diabetics: increased insulin sensitivity (not approved for this use)

Ulcerative Colitis Treatment Options. Coping with ulcerative colitis is no easy thing. The chronic disease, which may affect as many as 700,000 people in the United States, causes inflammation and sores in the lining of the colon and rectum. As the inflammation worsens, the cells that line these areas die off, resulting in bleeding, infection, and diarrhea. The condition can cause: *fever. *anemia. *fatigue. *joint pain. *appetite loss. *weight loss. *skin lesions. *nutritional deficiencies. *stunted growth in children. Cause. The exact cause of ulcerative colitis is unclear. Many experts believe it results from an abnormality of the immune system and an inability to handle bacteria in the digestive tract. Diagnosis. A doctor may request a blood test, stool samples, barium enema, and colonoscopy to determine whether your symptoms are being caused by ulcerative colitis rather than another condition like Crohn’s disease, diverticular disease, or cancer. Ulcerative colitis should be confirmed by biopsy (sampling) of tissue during colonoscopy. Treatment. If you’re diagnosed with ulcerative colitis, it’s vital to map out a treatment plan that controls and prevents attacks so that your colon can heal. Because the symptoms and effects of the disease vary, there’s no single treatment that works for everyone. Treatments often focus on diet and nutrition, stress, and medication. Diet and Nutrition. It’s best to consume smaller amounts of food throughout the day and avoid raw and high-fiber foods such as: *nuts. *seeds. *beans. *whole grains. Fatty and greasy foods also contribute to inflammation and pain. In general, safe foods include dairy, low-fiber grains, meats, and some raw fruits and vegetables. Sipping water throughout the day can aid in digestion and reduce inflammation. Stress. Anxiety and nervousness can worsen symptoms. Exercise and relaxation techniques that help you manage and reduce your stress levels can be helpful. These include: *biofeedback. *massages. *meditation. *therapy. Medication: A doctor may prescribe medication to induce or maintain remission. Although several types of medications exist, each drug falls into four primary categories. Aminosalicylates. These drugs contain 5-aminosalicyclic acid (5-ASA), which helps control inflammation in the intestine. Aminosalicylates can be administered orally, through an enema, or in a suppository. They typically take four to six weeks to work. However, they may cause side effects, including: *nausea. *vomiting. *heartburn. *diarrhea. *headache. Corticosteroids. This group of steroid drugs — including prednisone, budesonide, methylprednisolone, and hydrocortisone — reduce inflammation. They’re often used for patients suffering from moderate to severe ulcerative colitis, including those who haven’t responded favorably to 5-ASA drugs. Corticosteroids can be administered orally, intravenously, through an enema, or in a suppository. Side effects include: *acne. *facial hair. *hypertension. *diabetes. *weight gain. *mood swings. *bone mass loss. *increased risk of infection. Consequently, steroids are ideally used on a short-term basis to temper a flare up of ulcerative colitis, rather than as a medication that is taken every day to control symptoms. Sometimes, when ulcerative colitis is very severe, patients end up on a daily dose of steroids to maintain a normal lifestyle. Immunomodulators. These drugs, including azathioprine and 6-mercapto-purine (6-MP), reduce inflammation of the immune system — though they can take as long as six months to work effectively. Immunomodulators are administered orally and typically used for patients that don’t respond favorably to the combination of 5-ASAs and corticosteroids. Potential side effects include: *pancreatitis. *hepatitis. *reduced white blood cell count. *increased risk of infection. Biologics: These are a newer class of drugs used as an alternative to immunomodulators to treat people who haven’t responded well to other treatments. Biologics are more complex and target specific proteins. They can be given through intravenous infusion or injections. Currently, there are four FDA-approved biologic medications to treat UC: *adalimumab (Humira). *golimumab (Simponi). *infliximab (Remicade). *vedolizumab (Entyvio). Pharmacology nursing NCLEX review question on bronchodilators and corticosteroid administration for a patient with asthma. This NCLEX pharmacology review question will test your nursing knowledge about how to properly administer a bronchodilator and corticosteroid metered-dose inhaler. To answer this question correctly, you must know the mechanism of action of both the bronchodilator (Albuterol) and corticosteroid (Fluticasone). What is Albuterol? It is a short-acting bronchodilator. A bronchodilator (just as its name says) opens the bronchi and bronchioles of the lungs. When this happens more air is able to flow through the lungs. This is vital for the patient who has asthma because they experience bronchoconstriction. What is a Fluticasone? It is a corticosteroid used to prevent symptoms in patients with asthma. It will work by decreasing inflammation in the small airways. To administer these medications, the bronchodilator will be administered FIRST and then 5 minutes later the corticosteroids will be administered. The bronchodilator will open the airway first so when the corticosteroids are inhaled they can do their job within the lungs. It is important to remember that AFTER administering inhaled corticosteroids, the patient must gargle and rinse the mouth with water and spit (or a spacer can be used). The reason for this is because corticosteroids can cause thrush. #naturalhealthremedies What you eat can go a long way in managing your blood sugar level and insulin production. This is why for people suffering from diabetes eating the right food is of utmost importance. While there is a host of herbs and spices that can help treat diabetes, dill leaves, or suva bhaji or shepu, as it is known in India, can work wonders. Here is a diet plan diabetics can use to control blood sugar level. Traditionally, used as a flavouring agent for fish, egg and mushroom preparations, dill leaf curry is common in most Indian homes. According to Dr James Duke, an American botanist’s Ethnobotanical Database, dill has around 70 different chemicals that can help fight diabetes. Here are 10 other natural remedies to treat diabetes effectively. What is so special about dill? Studies have indicated that the leaf extracts have certain therapeutic properties that can help treat type 2 diabetes-induced due to the use of steroid, a condition commonly referred as corticosteroid-induce type 2 diabetes. When glucocorticoid therapy is used to treat other health conditions and reduce harmful inflammation in the body, its prolonged use can lead to hyperglycemia. A condition denoted by an excess of glucose in the bloodstream of a person leading to type 2 diabetes or diabetes mellitus. However, consumption of dill leaf extracts can help reduce the fluctuations in insulin levels and control blood sugar. Moreover, the leaf extract also helps in controlling thyroid functions and improve immunity. Here are 10 yoga poses that every diabetic should do. How to have dill to control diabetes In India, the most common way to consume this leaf is to make a curry with onions, garlic, ginger and green chilies with seasoning of mustard and cumin seeds. Alternatively, you can also make a juice with dill leaves. Put a bundle of the leaves, after thoroughly washing them, in a blender and blend at high speed. Next, strain the juice and squeeze half a lemon and add a pinch of black salt for taste. You can either have it in the morning or at night. You can prepare a dough with the dill leaf juice and make tasty and healthy paranthas. Else, use the leaf in soups, salads, pickles or curries for flavor. Chewing dill seeds can also help you in a similar manner or you use the seeds in tadkas (tempering). You can also make a powder dill seeds and use it as a masala in your dals and curries. Even if you don’t suffer from type 2 diabetes, eating dill can help you lower blood sugar levels and help in insulin management. So, say yes to this green leafy vegetable

Complementary and Alternative Treatments for Stroke. Know which risk factors for stroke are manageable: Blocked arteries, ruptured blood vessels, or blood clots can cause a stroke. Complementary and alternative medicine (CAM) may help with stroke prevention and recovery. CAM treatments can include massage, dietary supplements, or acupuncture to manage stress. Uncontrollable risk factors are: *age. *gender. *race. *a family history of stroke. *a personal history of stroke. If you think you’re having a stroke, call 911 or local emergency services. Common controllable risk factors for stroke include: *the use of alcohol or illicit drugs, such as cocaine. *smoking. *a lack of exercise or physical activity. *a poor diet. *an unhealthy weight. *diabetes. *stress. *depression. *poor cholesterol levels. *high blood pressure. *the use of nonsteroidal anti-inflammatory drugs, such as ibuprofen and naproxen, but not aspirin. According to a 50-year study of strokes in India, where strokes are more common than in Western countries, managing the risk factors for stroke was the best option for prevention. One-third of the people in their sample size made education and lifestyle changes to control their risk factors. These changes proved to be the most effective way to prevent another stroke in this population. Evidence doesn’t suggest that CAM treatments are better than medical treatments. CAM treatments shouldn’t replace any treatments your doctor has prescribed. But adding CAM treatments to your healthcare routine may help you reach health goals. For example, it may help lower your blood pressure faster. Check with your doctor first before taking CAM treatments. What to eat. Your doctor may prescribe medication for lowering cholesterol and may recommend a heart-healthy lifestyle. You should regularly eat or drink the following to help you reach your health goals. Black or green tea: Drinking at least three cups of black or green tea per day may help reduce your risk of stroke. Tea flavonoids, which are plant nutrients, can help decrease cholesterol and blood pressure. Researchers in one study found that people who drank that amount of green or black tea had far fewer incidences of repeated stroke. Black tea may be especially helpful for diabetes management. The compounds in black tea mimic the effects of insulin and prevent starch from turning into sugar. Fruits and vegetables. Eating more fruit and vegetables isn’t just good for your physical health. Researchers in a recent study found that eating more fruit may increase happiness and well-being as quickly as the next day. Increasing your intake to eight portions per day may increase life satisfaction and help lower stress levels. Pomegranate. Pomegranate concentrate is high in antioxidants and phytosterols, which are plant steroids that lower cholesterol. Adding pomegranate concentrate with low-dose statin therapy or the regular use of cholesterol-lowering drugs can help reduce cholesterol, according to the Israeli Institute of Technology. It may also lessen the statin’s side effects, such as muscle pain. Ways to start moving. Yoga is a good option for low-impact exercise. Tai chi is a Chinese exercise that practices slow and graceful movements in a semi-squatting position. Research shows that tai chi helps improve body balance and reduces depression and anxiety. Researchers in one study about tai chi and stroke prevention in seniors are currently exploring if tai chi has a role as a protective measure against ischemic stroke. Is Yuca Root Good or Bad For Diabetes Download Diabetes Management Book: http://bit.ly/2g0NDAH Hello, I'm Ty Mason from TheDiabetesCouncil.com, researcher, writer and I have type 2 diabetes. Today I'm going to answer the question, is yuca root good for diabetes. But before we get into that, make sure you download my free diabetes management book which also includes a diabetes grocery shopping guide (foods to eat and avoid). OK, first things first. When this question was posed, it was asked if the “YUCCA root” was good for diabetes. Right off the bat I can so NO! Why? Because the yucca is an ornamental spiky flowered plants common in Southern and Western parts of the US, including Florida, New Mexico, and California. But they don't have an edible root. So I think what is being asked about is the YUCA root. But don’t feel bad, spell check keeps trying to change yuca to yucca every time I type it and it is getting a little annoying. The yuca is a potato-like starch found in many Latino dishes and is also known as cassava. This is native to South American and is largely consumed throughout Latin America, Africa, South Asia and the Caribbean. The yuca got a very bad wrap in the diabetes community back in 1994 when a study published in the journal "Acta Horticulturae," suggested that cassava has been suspected of causing diabetes. However, several studies have shown a low incidence of diabetes in Africans who eat cassava regularly. A 2006 study found in the journal "Fundamental & Clinical Pharmacology," stated that none of the 1,381 subjects had diabetes, even though cassava accounted for a full 84 percent of their caloric intake. Another study, published in the October 1992 issue of "Diabetes Care," noted that Tanzanians who ate cassava regularly had a lower incidence of diabetes than those who rarely ate it. Yuca has a GI of 46 and a GL of 4. Both are low on their respective scales and indicate that yuca doesn’t affect the blood sugar greatly. So, is the yuca root good for diabetes? Yes it is. Just don’t confuse YUCA for YUCCA. There are parts of the yucca plant that are edible, mainly the flowers and fruit. We aren’t talking about fruit, we are discussing roots. One quick warning: you need to be very careful in eating yuca because of a high level of a toxic compound called hydrogen cyanide. This has the potential to worsen diabetes related circumstances. You can minimize your risk of toxicity by choosing a sweet-tasting variety of yuca. Cyanide levels can also be greatly reduced through soaking. I hope this answered your question is yuca root good or bad for diabetes. Don’t forget to get your diabetes management book. Let me know if you have any other diabetes related questions.

The boost to testosterone levels resulted in better control of blood sugar levels. About a third of men who have type 2 diabetes have low cholesterol levels. A common genetic problem among type 2 diabetic men whose ancestors came from Britain or Ireland is iron overload, which gradually shrinks testicular tissue so that their testosterone levels fall. Lower testosterone levels occurs at the same time as higher blood sugars levels. Men with type 2 diabetes should not attempt to raise testosterone levels with androstenedione. This popular supplement is proven to raise testosterone levels... but in women. One study found that taking no more than 300 mg of this supplement a day, might raise testosterone levels in older men. Unfortunately a similar dose raises estrogen levels in younger men. This is a supplement that men with type 2 diabetes are probably better off leaving alone. When carefully questioned, approximately 50 per cent of males with type 2 diabetes will admit to difficulty with their sexual function. This difficulty usually shows itself in the form of erectile dysfunction, which is the inability to have or sustain an erection that is sufficient for intercourse. Reasons besides insufficient production of the hormone testosterone include: * Overproduction of a hormone from your brain, called prolactin * Trauma to your penis * Medications to lower high blood pressure (anti hypertensives) and antidepressants * Poor blood supply to your penis due to blockage of the artery due to peripheral vascular disease The failure of erection in type 2 diabetics is is also determined by the degree of control of your blood sugar levels, better control of blood sugar levels is associated with fewer problems.
