A Patient who has been on Coumadin since the very beginning of 2010 is being trained on his second INR machine. He and his wife were trained on the first machine back in 2010 so that they would not have to make weekly trips to his physician or to an INR clinic to have his blood tested. They were trained and had to undergo several days of training to be able to do this at home. This video is them being trained for a second new machine which required much less blood and is much more streamlined than the first machine they had received 3 years ago. They only had to be shown once how to use this as they have been using the other machine for years and the testing procedure is very similar. 7. Performing PT INR Test

Make sure that you have all the supplies needed before you start, including, a 21 gauge lancet, Sample Transfer Tubes, sterile alcohol prep pads, gauze square, and a biohazard, or sharps, waste container. For fingerstick blood testing, increasing the flow of blood in the finger will help you capture a good drop of blood. Before you lance the finger, have the patient warm their hand by washing it in warm water, or by using a hand warmer. Ensure that the patient’s hand is dry before testing. Do not use fingers with tight rings, scars, calluses, or other features that prevent getting good access to the blood. One of the middle or index fingers on either hand is recommended. Gently squeeze or massage the finger to be lanced towards the tip of the finger. Clean the fingertip with an alcohol wipe using one side for the first cleaning. Use the second side for a final wipe. Dry the fingertip with gauze to remove any excess. Place the patient’s hand on a solid surface with the palm of the hand facing up. Remove the cap from the single use 21-gauge lancet. Place the lancet against the skin. Holding the body of the lancet, push down firmly against the finger to lance the surface of the skin. Collect the sample using a Sample Transfer Tube. Hold the transfer tube below the bulb and horizontal or with the bulb pointing slight towards the ground to let gravity assist the filling of the tube. Touch the tip of the tube to the bead of blood until the blood reaches the black line and stops moving. Do not squeeze the bulb of the transfer tube at any point during the collection process. Shift your thumb and index finger to the flat sides of the bulb on the transfer tube covering the air holes. Rest your hand on the meter. Roll your hand towards the test strip and touch the tip of the transfer tube down onto the bottom of the sample application well where the green light is flashing. Apply gentle pressure to the bulb until the blood just exits the tube. Do not produce air bubbles. Hold that pressure on the bulb while you pull your hand away from the meter to avoid back-suctioning the sample. You must apply the blood to the test strip within 15 seconds of lancing the finger. Once the sample transfer tube is filled apply the sample immediately to the test strip. The light should go out and the meter should display “Testing Please Wait”. The testing time is the patient’s actual clotting time as the system produces a true prothrombin time. Never add more blood to the test strip. Never collect a second sample from the same fingerstick. When testing is complete, the meter beeps once and the results (INR and Prothrombin Time appear on the screen. If an error is displayed, either not enough blood sample was applied or the sample had air bubbles in it. Refer to the trouble shooting section of the manual. Remove the strip and retest with a new strip and fresh fingerstick from a different finger. Remove the test strip and properly dispose along with the sample transfer tube and lancet. Turn off the meter. Record and report the result which can also be recalled from the memory. Turn the meter off by pressing the POWER button when you are finished testing. If left unattended, the meter automatically turns off in a few minutes. The last 100 test results are stored in memory with the time and date.

Coag-Sense PT/INR Monitoring System 1. The Coag-Sense PT/INR Monitoring system, with its direct clot detection technology, offers lab-quality results from the convenience of your home. 2. This video provides brief overview of how to perform a PT/INR self-test. Please be sure to read your entire user manual carefully before you use your system. 3. Layout out the required testing supplies in the order which they will be used. Note that home testing service providers typically do not provide alcohol wipes, in which case you should wash your hands with soap and warm water and dry them thoroughly before testing. If an alcohol wipe is used the finger must be completely dry as residual alcohol will affect results. 4. Press the ON/OFF button to turn the meter on. 5. Holding the meter steady with one hand, gently insert the test strip in a smooth fashion all the way to the back of the meter. The meter reads the barcode on the test which provides the information required to generate the INR result. If you receive an error on inserting the test strip, simply remove the strip and reinsert it in a smooth fashion. Do not discard the test strip. 6. While the test strip is warming, hold your hand below your heart and massage the middle finger from the palm of your hand towards the last joint of the finger. 7. After Apply Sample is displayed on the meter, press down hard on the finger with your lancet and lance your finger according to the instructions provided by your lancet manufacturer. Twenty-one gauge lancets are required from proper blood flow, do not use smaller diabetes lancets. 8. Apply pressure above the joint and squeeze the finger from the side using your thumb and index finger from your other hand. Keep squeezing the finger until you produce a pea sized bead of blood. 9. Hold the transfer tube below the bulb and horizontal or with the bulb pointing slight towards the ground to let gravity assist the filling of the tube. Touch the tip of the tube to the bead of blood until the blood stops moving. Keep squeezing your finger if necessary to produce sufficient blood to fill the capillary portion of the tube fills completely, approximately one inch from the tip of the tube. Do not squeeze the bulb of the transfer tube at any point during the collection process. 10. Shift your thumb and index finger to the flat sides of the bulb on the transfer tube covering the air holes. Rest your hand on the meter. Roll your hand towards the test strip and touch the tip of the transfer tube down onto the bottom of the sample application well where the green light is flashing. 11. Apply gentle pressure to the bulb until the blood just exits the tube. Do not produce air bubbles. Hold that pressure on the bulb while you pull your hand away from the meter to avoid back-suctioning the sample. This weeks video is a short guide on warfarin also known as (coumadin). I see many patients on warfarin everyday and they are very knowledgable about their medicine, which is fantastic! But here's some information about warfarin, how it works, international normalised ratio (INR) and interactions which you may find useful. WHAT IS WARFARIN: Warfarin is the main oral anticoagulant used in the UK. An anticoagulant is a medicine that prevents blood clotting. Clotting (thickening) is a complex process involving a number of substances called clotting factors. Clotting factors are produced by the liver and help control bleeding. They work with cells that trigger the clotting process (platelets) to ensure blood clots effectively. Warfarin blocks one of the enzymes (proteins) that uses vitamin K to produce clotting factors. This disrupts the clotting process, making it take longer for the blood to clot. WHEN IS WARFARIN PRESCRIBED: Anticoagulant medicines, such as warfarin, are often prescribed for people who've had a condition caused by a blood clot or have an increased risk of developing harmful blood clots. TAKING WARFARIN: It's very important that you take warfarin exactly as directed. Don't increase your prescribed dose unless the doctor in charge of your care advises you to. Warfarin is taken once a day, usually in the evening. It's important to take your dose at the same time each day, before, during or after a meal. The aim of warfarin therapy is to decrease the blood's tendency to clot, but not stop it clotting completely. This means the dose of warfarin you're taking must be carefully monitored and, if necessary, adjusted. You'll have regular blood tests at your GP surgery or local anticoagulant clinic to make sure your dose is correct. The INR is a measure of how long it takes your blood to clot. When you start taking warfarin, you may be given a yellow booklet about anticoagulants, which explains your treatment. INTERACTIONS WITH WARFARIN Medicines Warfarin can interact with many other medicines, herbal medicines and supplements. Always ask your pharmacist, GP or staff at your anticoagulant clinic before you take them as they may interact with your warfarin. Foods and drink Foods containing large amounts of vitamin K include: • Green leafy vegetables, such as broccoli and spinach • Vegetable oils • Cereal grains • Small amounts of vitamin K can also be found in meat and dairy foods. When your first dose of warfarin is prescribed, it doesn't matter how much vitamin K you're eating because the dosage will be based on your current blood clotting levels. However, if you make significant changes to your diet, such as increasing your vitamin K intake or cutting out foods that contain vitamin K, it could interfere with how warfarin works. Consult the healthcare professional responsible for your care before making any significant changes to your diet while taking warfarin. Why you should avoid cranberry juice whilst taking warfarin is in the link lower down. Alcohol Getting drunk or binge drinking is dangerous while taking warfarin. BLOOD GLUCOSE TESTING: Blood glucose testing, also known as blood glucose monitoring, is one of the main tools involved in controlling diabetes. Not everyone with diabetes will test their blood glucose levels but it is regarded as being very beneficial for helping to make diet and medication dosing decisions. If you are on any medication that can lead to hypoglycaemia (most notably insulin), you should test your blood glucose levels. WHAT IS A LANCING DEVICE: Lancing devices are used to obtain samples of blood for glucose testing using a lancet. A variety of lancets are on the market, the most common of which are automatic lancing devices (Used in the video). TESTING BLOOD GLUCOSE METER & TESTS STRIPS: Control solutions are used to check that the meter and test strips are working together properly and that the test is performing correctly. Most control solutions last 90 days after opening, please check yours for the actual date. The acceptable range of glucose for that solution is listed on the back of your test strip vial or control solution. USEFUL TIPS: Please watch the full video for all of them but here are some important ones, • Use a new lancet every time you use your lancing device. I know it's time consuming but if you don't change, it's going to hurt more and you're at risk of a skin infection. • Diabetic patients tend to build a collection of different blood glucose meters over time. It’s really important that you only ever use one meter to build up a trend of results because different meters will give slightly different results. HOW TO TEST BLOOD GLUCOSE: • Prepare your kit for testing. • This should include: your meter, test strip, lancing device, cotton wool, monitoring diary and sharps bin. • Ensure that the lancing device is primed with a new lancet. • Wash and dry your hands - to ensure that the result is not influenced by any sugars that may be present on your fingers. • A fuller drop of blood will be obtained if your fingers are warm, so it’s worth warming your hands by washing with warm water and rubbing them for 10 seconds. • Put a test strip into your meter, make sure it switches on and is ready. • Prick your finger with the lancing device at the sides of the finger as there are less nerve ending here than at the tips or the ‘pads’. Switch fingers regularly to prevent thickening of the skin. You may want to avoid using your little finger due to the skin being thin. • If your hands are warmed up you shouldn't need to squeeze your finger for a blood drop, if necessary apply light pressure to the surrounding area until a blood drop appears. Squeezing too hard can interfere with results. • Wipe away the first blood drop with clean cotton wool and use the second blood drop for testing (careful not to smear the drop). • Gently touch the blood drop with the test strip in the meter, wait a few seconds for result to appear. • If the test is successful, clean any blood off your finger – with the cotton wool if necessary. • Record the result/details in a monitoring diary. • Dispose of the test strip and ensure that the lancet used is put into a sharps bin.
