The good thing about having a nebulizer at home is if you are having an asthma attack every minute counts. There have been many times throughout my life that I had to get rushed to the hospital just to have them administer a breathing treatment. Nebulizers use oxygen, compressed air or ultrasonic power to break up medical solutions/suspensions into small aerosol droplets that can be direclty inhaled from the mouthpiece of the device. The definition of an aerosol is a "mixture of gas and liquid particles," and the best example of a natural occurring aerosol is "mist" (being formed when small vaporized water particles mixed with hot ambient air are cooled down and condense into a fine cloud of visible airborne water droplets). When using a nebulizer for inhalation therapy with medicine to be administered directly to the lungs, it is important to note that inhaled aerosol droplets can only penetrate into the narrow branches of the lower airways if they have a small diameter of 1--5 micrometers. Otherwise they are only absorbed by the mouth cavity, where the effect is low.

Coach Alicia: ED coding question: If someone comes in for asthma and was given just a nebulizer treatment (I’ve had that, it made me feel wonderful) can I charge just the nebulizer CPT code, or can I charge both that and E/M? We currently code only sutures with a straight laceration, so I think this follows the same protocol.” A: Well, if the doctor sees the patient in the ER, you get to code the E/M. Isn’t that right? You wanted to know if the treatment of the nebulizer treatment is covered under the E/M code or do you get to code both? I think that’s what they’re asking. You think so, Schuyler, or you’re getting a different vibe off of that? Coach Schuyler: For one thing, it doesn’t really say if the nebulizer controls that condition or the exacerbation of it. Or if it wasn’t exacerbation if that didn’t work, the status asthmaticus, or things like that. This could be like an opportunity for CDI just to make sure. Obviously it would be more documentation but it’s been a while since I’ve worked in ED, but there are times we bill for that facility or you bill for that pro fee. Coach Alicia: Jack is saying it’s probably fine with the modifier 25. It actually must be the one that offer that because about the laceration she goes, “So, the E/M is included in the suture if that’s the only problem.” In other words, it’s like a package deal, is what you’re talking about. Coach Schuyler: You want to look at your NCCI Edits to make sure and that’s where you would find all that information so you make sure that your not unbundling and things like that. Coach Alicia: Yeah. Since you already have a protocol that is for just a straight laceration, just suture, just we’re stitching that thing up. If there isn’t already a protocol in place, then maybe you could bring that up when you have your compliance meetings, which I’m assuming you guys have often because that’s what you should do and say, “Can we get a protocol in place” because quite honestly people coming in with asthma attacks and getting nebulizer treatments is a very common thing just as common as ear aches and lacerations.
