Today I am going to share with you simple way you could treat your knee, in case you’ve got an ligament injury, tendón tear or even meniscus tear. This home treatment is also good as a pain remedy for those who suffer from arthritis. Recently I’ve come across old homemade treatment for knee injuries which helped me quickly to restore my right knee. I love sports. I am practicing Brazilian Jiu-Jitsu and two weeks ago I got serios injury during the training . It was an collateral ligament injury. I thought it is over. The most common cause of ACL ruptures: •Traumatic force being applied during twisting motion. •Side stepping or landing from a jump. Patient complains of: •Immediate pain •Knee giving way •Swelling Aspiration of the knee •If aspiration of the knee joint shows evidence of blood within the joint there is 75-80% chance of ACL and meniscal injury. Lachamn’s test- ACL knee exam •Knee is flexed at 30 degrees. •ACL tear of the knee is identified by pulling on the tibia and examining the frontward motion of the lower leg in comparison to the upper leg. Radiological exam – ACL •MRI of the knee joint shows bone lesions or bruising associated with tears of the ACL. Injury is found in the typical location; middle of the femoral condyle and posterior part of the tibia laterally. Posterior cruciate ligament tear (PCL) •Common cause of injury is a bent knee hitting a dashboard in a car accident. Tibial Sag Test –PCL knee exam Quadriceps active test-PCL knee exam •The examiner stabilizes the leg of the patient and then the patient is asked to actively contract the quadriceps muscle. •The tibia is seen actively reduced from the posterior subluxed position. Lachman’s test-PCL knee exam •Knee is bent 20-30 degrees. •The posterior drawer test is carried out while the patient is in a supine position and the knee is flexed to 90 degrees. •The amount of translation of the tibia relative to the femur is observed. The dial test is performed while the patient is in the supine or prone position and both knees are in 90 and 30 degrees of flexion. Within the cartilage matrix, there are numerous prototype and monomers attached by link protein to a molecule of hyaluronic acid. This is all woven with collagen to form an elastic and compressible structure. Normal articular cartilage also known as hyaline cartilage provides a smooth crystal clear gliding surface to aid in the motion of the joint. The structure of hyaline cartilage is divided into four layers; superficial, middle, deep and calcified layer with the calcified layer representing a transition from articular cartilage to bone. This cartilage is designed to achieve and maintain proper function over the majority of a person’s life. If the cartilage is subjected to excessive weight, overuse, improper alignment or injury, it begins to wear away leaving bone to rub on bone. Articular cartilage does not have the ability to heal itself, however, there are several treatment options available to aid in the repair of injured cartilage. The knee is the most common joint in the body with cartilage injury issues. The knee joint is composed of femur, tibia, and patella. There are two types of cartilage in the knee joint, the articular cartilage and the meniscus, which is a shock absorbing cartilage. There are also important ligaments within the knee joint, the anterior and the posterior cruciate ligaments. The goal of treating cartilage injury is to reduce pain and improve the function of the affected area. Anti-inflammatory medications are commonly used to combat-injured cartilage but they can potentially cause cardiovascular or gastrointestinal toxicity. Damaged cartilage can also be treated with a series of injections. Steroid injections such as cortisone are powerful anti-inflammatory drugs used in the treatment of osteoarthritis. Hyaluronic acid is a substance produced naturally in the body and is present within cartilage when isolated. Hyaluronic acid is a thick viscous solution that can be added to the natural hyaluronic acid of the knee joint. This solution uses purified Hyaluronic acid derived from either rooster combs or genetically engineered cells. Glucosamine is another option for treatment that can be injected directly into the joint. Glucosamine is a modified sugar that is formed by the human body and is used to form larger molecules involved in the formation and repair of cartilage. Other methods include gene therapy, platelet-rich plasma, growth factors, stem cells, drilling and debridement of cartilage may help with injured cartilage. Occasionally the cartilage is replaced, regrowth and implanted. When the cartilage is damaged, the choice is often simple as the patient will need a total knee replacement. Many different types of designs and materials are currently used in total knee replacement surgery. Nearly all consists of three components, a patellar component, which consists of durable plastic, a tibial component, made of a durable plastic cushion often held within a metal platform and the femoral component, which is usually made of highly polished metal. Total knee replacement is the most predictable option to treat severe arthritis of the knee joint. When the arthritis is minimal, almost all treatment methods will work, however, when the extent of arthritis is moderate, there are many different treatment options presented, but none of them are predictable. It is logical to pursue and recognize the efficacy of platelets and stem cells in cartilage regeneration, simply because it is better for the patient to be able to regenerate their own cartilages and for it to be replaced with an artificial joint.

Dr. Ebraheim’s educational animated video describes test for evaluation of knee injuries and all the maneuvers you need to know to preform an excellent knee examination. McMurray’s test is a knee examination test that elicits pain or a painful click as the knee is brought from flexion to extension with either internal or external rotation. The McMurray’s test uses the tibia to trap the meniscus between the femoral condyle and the tibia. When performing the McMurray’s test, the patient should be lying supine with the knee flexed. The examiner grasps the patient’s heel with one hand and places the other hand over the knee joint. To test the medial meniscus, the knee is fully flexed and the examiner then passively externally rotates the tibia and places a valgus force. The knee is then extended in order to test the medial meniscus. to tests the lateral meniscus, the examiner passively internally rotates the tibia and places a varus force. The knee is then extended in order to test the lateral meniscus. A positive test is indicated by pain, clicking or popping within the joint and may signal a tear of either the medial or lateral meniscus when the knee is brought from flexion to extension. Lachman’s test is the most sensitive and best test for examining an ACL injury. The patient should be lying supine and completely relaxed. Make sure that the patient’s hip, quadriceps and hamstring muscles are all relaxed. Bend the knee to about 20-30°. Stabilize the femur with one hand and with the other hand, pull the tibia anteriorly and posteriorly against the femur. With an intact ACL as the tibia is pulled forward the examiner should feel an endpoint. If the ACL is ruptured, the ACL will be lax and the examination will feel softer with no endpoint. The tibia can be pulled forward more than normal (anterior translation). Both the Lachman’s test and the Pivot shift test are associated with 20-30°s of knee flexion. The Lachman’s test starts at 20-30 ° of flexion. With the Pivot shift test you feel the clunk at 20-30°s of flexion. 20-30°s of flexion is important for examination of the ACL (remember that). The patient should be lying supine. Make sure the patient is totally relaxed. With pivot shift, the knee is in the subluxed position and the knee is in full extension. The pivot shift starts with extension of the knee and you can feel the clunk at 20-30° of flexion. Hold the knee in full extension then add valgus force plus internal rotation of the tibia to increase the rotational instability of the knee. Then take the knee into flexion. A palpable clunk is very specific of an ACL tear. the iliotibial band will reduce the tibia and create the clunk on the outside of the knee. Always compare with the other side. The reverse pivot shift test helps to diagnose acute or chronic posterolateral instability of the knee. A significantly positive reverse pivot shift test suggests that the PCL, the LCL, the arcuate complex and the popliteofibular ligament are all torn. The reverse pivot shift test begins with the patient supine with the knee in 90° flexion. Valgus stress is then applied to the knee with an external rotation force. Bring the knee from 90°s of flexion to full extension. The tibia reduces from a posterior subluxed position at about 20°s of flexion. A shift and reduction of the lateral tibial plateau can be felt as it moves anteriorly from a posteriorly subluxed position. A clunk occurs as the knee is extended. This is called reverse pivot shift because shift of the lateral tibial plateau occurs in the opposite direction of the true pivot shift (Seen in ACL tears). If the tibia is posterolaterally subluxed, the iliotibial band will reduce the knee as the IT band transitions form a flexor to extensor of the knee. It is very important to compare this tests to the contralateral knee. Pivot shift = ACL tear The test is done with the patient in supine position and the knee is flexed to 90°. The examiner stabilizes the foot. Next the examiner pushes backward on the tibia, looking for the tibia to sag posteriorly. Observer the sag that develops due to tear of the posterior cruciate ligament (PCL). The amount of translation in relationship to the femur is observed. The test is considered positive if excessive posterior translation of the tibia is demonstrated. DR Nabil Ebraheim UTMC Toledo. The cartilage of the knee is complex and it is made of elastic comprehensive structure. The normal articular cartilage is called hyaline cartilage; it provides a smooth, gliding surface to help the motion of the joint. There is about 2cc of the synovial fluid inside the knee that helps in the motion and lubrication of the joint. Between the hyaline cartilage which is called the articular cartilage you can see the meniscus, the lateral and the medial, the meniscus is a shock absorbing cartilage or cushion between the articular cartilages. The hyaline cartilage has 4 layers: 1- The superficial layer. 2- The middle layer. 3- The deep layer. 4- The calcified layer After the calcified layer you find the bone. These cartilage cells are supposed to live forever. Good cartilage cells are sterile; they can’t make more cartilage if these cartilage cells are destroyed. If the cartilage is subjected to excessive wear, trauma, injury, overuse, excessive weight or improper alignment, then the cartilage will wear away leaving the bone to rub against bone. The cartilage doesn’t have the ability to heal itself by hyaline cartilage, but sometimes it can heal itself by inferior type of cartilage called fibrocartilage, especially if the area that needs to be repaired is small. What is the treatment of the arthritis of the knee? 1- Medication: people respond differently to medications, the doctor select the type, dosage and duration of the treatment, the medication has to be safe and effective. Ask the doctor about the side effect of the medication. There are several medications for example: NSAIDS. 2- Losing Weight: it’s advisable to lose about 5% of the body weight, especially if the BMI was above 25. 3- Low Impact Physical Therapy: such activities like swimming or cycling, adds less stress on the knees, life style modification to protect the knee will slow the progress of the arthritis, there is a strong evidence that physical therapy will help the patient. The physiotherapy will decrease the pain, will improve the function, increase the strength, the range of motion; physiotherapy should be individualized with program that meets the patient’s needs, the life style, and expectations. 4- The physician will also use intra-articular injection of steroids; viscosupplementation, or hyaluronic acid, injection will relieve the patient’s pain and disability. 5- Other injection methods: - Gene therapy - PRP - Growth factor - Stem cells 6- Other treatment options: - Acupuncture - Massage - Glucosamine - Chondroitin sulfate - Valgus directing brace - Wedges in the foot - Arthroscopic debridement and lavage 7- Assistance devices: - Cane - Shock absorbing shoe - Shoe inserts - Knee sleeves - Support brace 8- Arthroscopy shouldn’t be done in arthritis unless there is loose body or meniscal tear causing mechanical symptoms of recurring lacking, catching, swelling and pain. There are some guide lines from the American Academy of Orthopaedic Surgeon AAOS: these guidelines are suggestive, and the treatment of the arthritis patient should be individualized and based on: - The doctor’s clinical judgment. - The patient’s clinical situation. - The evidence tested and published. It is a combination of all of these factors. If it works for you, it may not work for your neighbor. So basically arthritis can be minimal, and anything can help the arthritis ion this situation. But if the arthritis is moderate then the physician has many options for treatment and none of those options are predictable in the result. When the arthritis is severe, total knee is the most predictable option for treatment of arthritis. You have to reduce the pain and improve the function of the knee. Patients with early stage of Knee Arthritis come to me at the age of 35 to 40 years, they are mainly youngsters who are gone to the gym or to a trek and they come with knee pain. There are another group of youngsters who come with rheumatoid arthritis, a little bit younger may be 30 to 35 years. These patients often come with pain on the inner aspect of the knee, swelling on the knee, a little bit of stiffness. So these are the main symptoms of Knee Arthritis. The main treatment for this early Arthritis is, it can be depicted by the mnemonic RICE meaning rest, ice, compression and elevation. Rest means stop the patient from doing those activities for 2 to 3 weeks Which gives them comfort. Ice the part or knee for around 10 to 15 minutes duration, 3 to 5 times a day with the help of gel packs, give some sort of compression as in crape compression and some amount of elevation this along with some amount of anti inflammatory medication and Physiotherapy should help the patient get back to his normal activities. One should know that this early Arthritis is quite common among youngsters. It is seen between 35 to 40 years because that's the age when the degeneration of the knee starts or setting it. So after a trek, a hill climb or a marathon these sort of pains are quite common there is nothing to get worried about this phenomenon.

For instance, a patient that presents with lateral knee pain has an MRI that shows severe cartilage degeneration of the lateral compartment, but the X-rays still show a gap. The Anterior Cruciate Ligament (ACL) helps keep the knee stable, and prevents it from sliding forward. When you have a Torn ACL, your knee can become painful and unstable. These stretches and exercises are for a non-operative or pre-operative injury. See Doctor Jo’s blog post about this at: http://www.askdoctorjo.com/acl-tear-stretches-exercises If you have an ACL surgery, make sure you follow your doctor and physical therapist protocols because there are several different ways to do the surgery, and they require different rehab during the recovery. The first exercise is going to be a heel slide. Sit on the ground or your bed with the leg you want to exercise straight out in front of you. You can bend the other one up to a comfortable level or leave it all the way down. Slide your leg up keeping your foot flat on the ground. Go as far as you comfortably can, and then slide back down. Start with 10 of these, and work your way up to 20-25. Now you will do a quad set. Put your leg you want to exercise out in front of you. You want to squeeze or activate the quad muscle by pushing your knee down into the ground. If you want to roll up a small towel to place underneath your knee for a target, you can. Also, pulling your toes up towards you will help straighten out the knee and contract the muscles. Hold this for 3-5 seconds and relax. Start with ten of these and work your way up to 20-25. Then you will do an ankle pump. You can prop your ankle up on something if you want, but keep your heel free to move around. I am using a roll, but you can just hang your foot off of the bed or stool. You are going to pull your toes up at your ankle, and then push down like you are pushing on a pedal. This is dorsiflexion and plantarflexion. It works as a natural pump to get the fluid and inflammation out of the leg, so you can do this several times a day. Next you will do a short arc quad (SAQ). For this, you will use a foam roll or a rolled up beach towel. You want something big so you have a good bend in your knee. Prop your leg over the roll with your knee on the roll. Try to keep your knee on the roll the whole time, and then lift your foot in the air until your leg is straight. If you pull your toes towards you at the end, this will help activate all the muscles in the leg. Try not to go fast because then you are only using momentum. Go slow and controlled with your movements. Start of with 10 of these, and then work up to 20-25. Now you will do a straight leg raise (SLR). Lie down on your back, and bend one knee up. Straighten out the leg you want to exercise. You can prop up on your elbows or sit up, but this will make it harder. Pull your toes toward you to help lock out the knee. Just lift your leg to the height of your bended knee. There is no need to kick your leg way up in the air, and it actually takes away from the exercise. Make sure you are slow and controlled with this exercise. Going fast and using momentum will not be effective. If you get to 20-25 and it is easy, then you can add weights if you have them. Finally, you will do some knee props. You can use a roll to prop up your leg, or you can sit in a chair, and have something of equal height in front of you. It can be another chair, an ottoman, etc. Place your heel on the other chair, and try to straighten out your knee as much as possible. If this is not enough stretch, you can add weights to it. A sack of potatoes works well, but sometimes it is too much weight to start with on your leg. Get a plastic bag, and add some soup or vegetable cans to it. Slide your foot through the loop of the bag, and place it just above your knee. Try to hold it there for 5-10 minutes and relax as much as you can. Over time, you can add more cans of soup. There is two types of cartilage between the femur and tibia: • Articular cartilage • Meniscus cartilage And there is a meniscus in between. The meniscus is a fibrocartilage structure that carries type I collagen, it is triangular shaped in cross section. There are two menisci: • Lateral meniscus:more mobile that the medial meniscus - Circular - Covers about 70% of the lateral tibial plateau • Medial meniscus: its tears occur about three times more than that of the lateral meniscus - C-shaped - Covers about 50% of the medial tibial plateau Function: • Shock absorption • Load sharing • Joint stability Meniscal tear causes: • Twisting, jumping, or changing directions during sports activities such as skiing or football. • Degenerative tears of the medial meniscus may occur in older patients. • Degenerative tears may occur in association with arthritis. Symptoms of the meniscal tear: - Pain on the medial or lateral side of the knee Mechanical symptoms: - Locking - Clicking - Swelling Examination: Joint line tenderness is the most sensitive exam; posterior knee pain may also be present when the knee is bent. Look for the knee joint effusion, effusion may be difficult to find and occur several hours to appear after the injury. With ACL tears, the hemorrhage and swelling is greater and develops rapidly. Mcmurray’s test is used to diagnose meniscal tear, a painful pop or click is obtained as the knee is brought from flexion to extension with either internal or external rotation of the knee. To test the medial meniscus, flex the knee and place the hand on the medial aspect of the knee. The knee is then extended in order to test the medial meniscus. Feel a pop or click with knee extension, the patient will experience pain. A positive test is indicated by pain, clicking, or popping within the joint and may signal a tear of the medial meniscus. To diagnose the lateral meniscus tear, do the Mcmurray’s test with internal rotation of the knee. Sometimes the knee is locked with lack of full extension due to a bucket handle tear of the meniscus. Differential diagnosis: • The clinical diagnosis accuracy of meniscal tear is about 70%. • MRI is usually done to confirm the diagnosis or identify other problems in the knee. • The differential diagnosis can be intra-articular problems or extra-articular problems. • Intra-articular problems: - Medial synovial plica irritation. - Osteochondritis lesions (OCD). - Patellofemoral pain - Loose bodies • Extra-articular problems: - Collateral ligament injury, especially the medial collateral ligament - Pes anserine bursitis - Lumbar disc herniation - Stress fracture - Iliotibial band syndrome - Slipped capital femoral epiphysis (SCFE). Blood supply: The meniscus receives blood supply from the genicular vessels and its capsular attachment. The peripheral 1/3 area is the most vascularized area and tears within this area usually heal. Where is the tear located? Treatment of meniscal tears: • Nonoperative: - Especially for small nondisplaced tears and degenerative tears. - Physiotherapy - NSAIDS - With persistence of symptoms, cortisone injection may be given. - When the conservative treatment fails, then do surgery. • Surgery: - Excision of the tear (partial meniscectomy): you do excision if the tear is complex, degenerative, or if it is a radial tear that cannot be repaired. - Repair of the meniscal tear: usually done for a peripheral tear which is vascular and will heal. It is better to have the combination of repair of the meniscus at the same time you do ACL reconstruction (controversial). - Meniscal transplant: done in younger patients who had total meniscectomy (especially lateral meniscus). It takes about one year for the graft to heal. Re-tear of the transplanted meniscus is also common. If you do total meniscectomy, this will probably lead to future arthritis of the knee. Special situations: • In general, medial meniscal tears occur more often than tear of the lateral meniscus. - In older patients, the posterior horn of the medial meniscus is usually more often affected. • With ACL tears that are acute, then there will be more incidences of lateral meniscal tears. • With ACL tears that are chronic, then there will be more incidences of medial meniscal tears. • With tibial eminence fractures in children, the medial meniscus can become trapped in this injury. • Tibial plateau fractures: - If the injury is medial then it can affect the medial meniscus. - If the injury is lateral then it can affect the lateral meniscus. • If you have depression or displacement separation more than 5 mm, you can have meniscal injury. Discoid meniscus: the meniscus is usually larger than normal and occurs more in the lateral meniscus, the treatment is usually saucerization plus or minus repair of the lateral meniscus if symptomatic.

They are connected with ligaments, tendons, and cartilage to each other and the surrounding muscles. The knee joint is the largest in the human body. It handles a tremendous amount of stress as it allows lower body mobility while supporting the upper body. Over time, the cartilage at the end of the bones that form the joint can deteriorate. In addition, there are fluid sacs in the knee capsule to keep the joint lubricated. These bursae can become irritated or inflamed if injured, resulting in bursitis. There are many potential causes for knee joint pain: Osteoarthritis – a chronic condition that occurs when the cartilage between bones breaks down Rheumatoid arthritis – an autoimmune disease that affects joints, causing swelling Bursitis – inflammation of fluid sacs either in front or directly behind the kneecap Immuno-inflammatory conditions Injury/over-use Infection Tendonitis – inflammation of tendons Gout – a form of arthritis in which uric acid accumulates and crystals of it settle in a joint Fortunately for our knees, there are natural remedies to reduce inflammation and ease the pain. The following recipe is a knee joint pain relieving rub. It contains effective anti-inflammatories and analgesics. Let’s look at what’s in it. 1. Cayenne. Cayenne has been used by Native Americans for millennia for pain relief. Capsaicin is a phytochemical in this spicy pepper that relieves pain through a variety of mechanisms. First, capsaicin blocks chemical pain transmitter “substance P” from reaching the brain. Second, when applied topically, it penetrates the skin to defunctionalize fibers that respond to pain stimuli. Through the skin, capsaicin stops nerves fibers from feeling pain and transmitting it. The capsaicin in cayenne is also effective for inflammatory skin conditions like psoriasis and eczema. Cayenne works to relieve inflammation when taken internally as well. 2. Apple Cider Vinegar. Supremely beneficial for human health in a wide range of applications, ACV is a potent anti-inflammatory that can relieve joint stiffness. The acetic acid in ACV penetrates the skin and can help to break up acid crystals that can form in the joint. Acetic acid has also been found to provide effective relief for tendonitis and other joint inflammation. (2) Additionally, unfiltered ACV contains bone-supporting minerals calcium, magnesium, potassium, and phosphorus. 3. Olive Oil. Anti-inflammatory and emollient, olive oil is a wonderful carrier oil. 4. Ginger. A potent anti-inflammatory, the phytochemical gingerol inhibits excessive immune response that results in inflammation. A 2014 study of the daily topical application of ginger on joints of people with osteoarthritis (OA) resulted in pain relief within a week and continued for the 24-week duration of the study. Natural Rub Recipe for Bone, Knee and Joint Pain You can apply this paste on any joint to reduce inflammation and relieve pain. You may want to experiment with the recipe and mix half the amount of ACV with the olive oil; sometimes the rub can be quite strong and the olive oil will lessen heat and make the paste easier to apply. All of these ingredients will also reduce inflammation if you eat them. Please note that cayenne can stain fabrics, so take care around adjacent clothing.
