The process is the opposite of vasodilation, the widening of blood vessels. The process is particularly important in staunching hemorrhage and acute blood loss. When blood vessels constrict, the flow of blood is restricted or decreased, thus retaining body heat or increasing vascular resistance. This makes the skin turn paler because less blood reaches the surface, reducing the radiation of heat. On a larger level, vasoconstriction is one mechanism by which the body regulates and maintains mean arterial pressure. Medications causing vasoconstriction, also known as vasoconstrictors, are one type of medicine used to raise blood pressure. Generalized vasoconstriction usually results in an increase in systemic blood pressure, but it may also occur in specific tissues, causing a localized reduction in blood flow. The extent of vasoconstriction may be slight or severe depending on the substance or circumstance. Many vasoconstrictors also cause pupil dilation. Medications that cause vasoconstriction include: antihistamines, decongestants, and stimulants. Severe vasoconstriction may result in symptoms of intermittent claudication. The mechanism that leads to vasoconstriction results from the increased concentration of calcium (Ca2+ ions) within vascular smooth muscle cells. However, the specific mechanisms for generating an increased intracellular concentration of calcium depends on the vasoconstrictor. Smooth muscle cells are capable of generating action potentials, but this mechanism is rarely utilized for contraction in the vasculature. Hormonal or pharmokinetic components are more physiologically relevant. Two common stimuli for eliciting smooth muscle contraction are circulating epinephrine and activation of the sympathetic nervous system (through release of norepinephrine) that directly innervates the muscle. These compounds interact with cell surface adrenergic receptors. Such stimuli result in a signal transduction cascade that leads to increased intracellular calcium from the sarcoplasmic reticulum through IP3-mediated calcium release, as well as enhanced calcium entry across the sarcolemma through calcium channels. The rise in intracellular calcium complexes with calmodulin, which in turn activates myosin light-chain kinase. This enzyme is responsible for phosphorylating the light chain of myosin to stimulate cross-bridge cycling. Once elevated, the intracellular calcium concentration is returned to its basal level through a variety of protein pumps and calcium exchangers located on the plasma membrane and sarcoplasmic reticulum. This reduction in calcium removes the stimulus necessary for contraction, allowing for a return to baseline. Factors that trigger vasoconstriction can be of exogenous or endogenous origin. Ambient temperature is an example of the former. Cutaneous vasoconstriction will occur because of the body's exposure to the severe cold. Examples of endogenous factors include the autonomic nervous system, circulating hormones, and intrinsic mechanisms inherent to the vasculature itself (also referred to as the myogenic response).

This vasoconstrictive effect reduces swelling ,inflammation and mucus formation in the nasal passages and makes it easier to breath.The bad part about many decongestants is that they don't only make the vessels in your nasal passages smaller but they affect the vessels in your entire body as well which can lead to an elevated blood pressure. For Americans that already have high blood pressure, this could be dangerous. Common decongestants are Phenylephrine and pseudoephedrine. Sometimes these medications are kept behind the pharmacy counter. They are not prescription medications so you just need to ask your pharmacist for them. Many of the multisymptom cold and flu concoctions available on the market today have one of these decongestants in them. It's important to look at the ingredients and understand what you are taking so that you don't overdose and so that you know how your body responds to certain ingredients. Decongestants can make some people feel jittery or have trouble sleeping. Those with high blood pressure may want to steer clear of oral decongestants. You should only treat the symptoms you have. Any and all medications carry with them possible risks and side effects. That being said, if you don't treat some cold symptoms they may worsen. This lesson discusses responding to distributive shock. The three types of distributive shock discussed are septic, anaphylactic and neurogenic. The approaches are different, according to the type of distributive shock, and those differences are highlighted within the lesson. There is also information given about the administration of medicines and broader evaluation. "The initial management of distributive shock is to increase intravascular volume. The intent is to provide enough volume to overcome the inappropriate redistribution of existing volume. To do so, administer 20 mL/kg of fluid as a bolus over 5 to 10 minutes and repeat as needed. Beyond initial management, therapy is tailored to the cause of the distributive shock. In septic shock, aggressive fluid management is generally necessary. Broad-spectrum intravenous antibiotics are a key intervention and should be administered as soon as possible. In addition, a stress dose of hydrocortisone (especially with adrenal insufficiency) and vasopressors may be needed to support blood pressure. After fluid resuscitation, vasopressors are given if needed and according to the type of septic shock. Normotensive individuals are usually given dopamine; warm shock is treated with norepinephrine; and cold shock is treated with epinephrine. Transfusing packed red blood cells to bring hemoglobin above 10 g/dL treats decreased oxygen carrying capacity. As blood cultures return, focus antibiotic therapy to the particular microbe and its resistance patterns. For anaphylactic shock, intramuscular epinephrine is the first and most important treatment. In severe cases, a second dose of epinephrine may be needed or intravenous administration may be required. Crystalloid fluid can be administered judiciously. Remember that in anaphylactic shock, capillary permeability may increase considerably. Thus, while it is important to support blood pressure overall, there is significant likelihood that third spacing and pulmonary edema will occur. Antihistamines and corticosteroids can also blunt the anaphylactic response. If breathing challenges arise, consider albuterol use to achieve bronchodilation. In very severe cases of anaphylactic shock, a continuous epinephrine infusion in the Neonatal Intensive Care Unit, or NICU, or Pediatric Intensive Care Unit, or PICU, may be required. Neurogenic shock is clinically challenging because often there is limited ability to correct the insult. Injury to the autonomic pathways in the spinal cord results in decreased systemic vascular resistance and hypotension. An inappropriately low pulse or bradycardia is a clinical sign of neurogenic shock. Therefore, treatment is focused on fluids first. Administer 20 mL/kg bolus over 5 to 10 minutes; then reassess the individual for a response. If hypotension does not respond to fluid resuscitation, vasopressors are needed. This resuscitation should be done in conjunction with a broader neurological evaluation and treatment plan."
