How Is Liver Cancer Diagnosed?

Once a suspicion of liver cancer arises, a physician will order one the following: Blood tests: alfa-fetoprotein (AFP), which may be elevated in 70% of patients with liver cancer. AFP levels could be normal in liver cancer. A rising level of AFP is suspicious for liver cancer. Other labs tests include des-gamma-carboxy prothrombin, which can be elevated in most patients with liver cancer. Imaging studies: Multiphasic helical CT scan and MRI with contrast of the liver are the preferred imaging for detecting the location and extent of blood supply to the cancer. If any imaging study is inconclusive, then an alternative imaging study or follow-up imaging study should be performed to help clarify the diagnosis. Lesions smaller than 1 cm are usually difficult to characterize. Liver biopsy is performed to sample tissue from the lesion in the liver, which is analyzed by a pathologist to confirm the suspected diagnosis of liver cancer. Liver biopsy is not needed in every case, especially if the imaging study and lab markers are characteristic for liver cancer. Risks of liver biopsy are infection, bleeding, or seeding of the needle track with cancer. Seeding is when cancer cells get on the needle used for a biopsy and spread to other areas touched by the needle. Liver biopsy of suspected liver cancer carries the added risk of seeding the liver biopsy needle track in 1%-3% of cases. AFP BLOOD TEST Purpose, Procedure & Result OVERVIEW An alpha-fetoprotein (AFP) test is a blood test that measures the amount of AFP present in blood. It’s usually part of what’s called a triple screen or quad screen in the second trimester of pregnancy. However, it can also be useful for adults who aren’t pregnant. The yolk sac, GI tract, and liver of an unborn baby produce AFP. It then circulates through the fetal and maternal blood. Individuals who aren’t pregnant still have some AFP in their blood, but levels are normally low. High levels of AFP in adults who aren’t pregnant usually indicate certain types of liver disease. PURPOSE An AFP test is a routine screening test that’s given to expectant mothers between the 14th and 22nd weeks of their pregnancy. It is most accurate between the 16th and 18th weeks, so it’s important to know exactly when you became pregnant. AFP testing is usually part of a quad screen. This screening exam also tests your levels of: • Human chorionic gonadotropin (HCG). • estriol, which is a hormone produced by your placenta and your baby’s liver. • inhibin A, which is a hormone produced by your placenta. Doctors will use your quad screen results, your age, and your ethnicity to help determine the chances that your unborn baby has a genetic birth defect. Defects detected by this type of screening can include neural tube defects, such as spina bifida, and chromosomal abnormalities, such as Down syndrome. The AFP results will help your doctor determine if you need further tests for these conditions. A positive test doesn’t necessarily mean your unborn baby will have a birth defect. The AFP test is especially important for women who are at high risk of having children with birth defects, including women: • Who are 35 or older. • With a family history of birth defects. • Who used harmful medications or drugs during their pregnancies. • Who have diabetes. If you aren’t pregnant, an AFP test can help to diagnose and monitor certain liver conditions, such as liver cancer, cirrhosis, and hepatitis. It can also help detect several other cancers, including cancers of the: • Testes. • Ovaries. • Biliary tract. • Stomach. • Pancreas. PROCEDURE You’ll need to get your blood drawn for an AFP test. Having blood drawn is an outpatient procedure that’s usually performed at a diagnostic lab. The procedure takes only a few minutes and is relatively painless. No special preparation is necessary before an AFP test. A healthcare professional will use a small needle to withdraw blood from a vein, usually in your arm or hand. A laboratory specialist will analyze the sample. Results are typically available within one to two weeks. RESULTS For women who aren’t pregnant as well as men, the normal amount of AFP is usually less than 10 nanograms per milliliter of blood. If your AFP level is unusually high but you aren’t pregnant, it may indicate the presence of certain cancers or liver diseases. If you’re pregnant you have higher-than-normal AFP levels, it can indicate a neural tube defect in your developing baby. However, the most common cause of elevated AFP levels is inaccurate dating of the pregnancy. AFP levels vary widely during pregnancy. The test will be inaccurate if you’ve been pregnant for a longer or shorter period of time than you thought. If you’re pregnant and your AFP level is unusually low, it could indicate that your fetus has a chromosomal abnormality, such as Down syndrome or Edwards syndrome. You can have an abnormal AFP reading due to a multiple pregnancy, such as having twins or triplets. An abnormal AFP reading can also be due to fetal death.

We start with alcohol, the main driver of liver damage. Now alcohol can damage your liver through many different mechanisms, including increasing fat synthesis, creation of free radicals, and toxic byproducts. Now minor drinking can lead to hepatic steatosis, which is just some fatty buildup. However, if you keep drinking, than it can lead to inflammation (or hepatitis). This leads to destruction and Mallory bodies. Eventually, it can lead to alcoholic cirrhosis and fibrosis. Again this affects zone 3! Now there is something called nonalcoholic fatty liver. Judging from the name it's from metabolic syndrome and not alcohol! Labs will be different depending on the etiology. Moving on, let's talk about Reyes syndrome. This occurs when a child is given aspirin and can lead to mitochondrial dysfunction, hypoglycemia, hepatomegaly. Now let's talk about hereditary liver diseases. This is a group of inherited liver disease due to a buildup of some toxic substance. Suspect this in a young patient with cirrhosis who doesn't drink. Hereditary liver diseases include: Alpha-1 antitrypsin deficiency: A1AT is made in the liver and goes to the lungs and protects it from neutrophilic elastase. Deficiency is codominant and leads to cirrhosis and panacinar emphysema. This is from misfolded proteins that is PAS positive. Wilson's disease: this is a mutation in ATP7B, which helps you excrete copper. Normally copper is excreted via bile or it binds to a protein called ceruloplasmin. When disrupted, copper is not free to deposit anywhere, including the liver, the eyes (kayser fleischer rings), and brain like the basal ganglia (Parkinsons and dementia). Labs include increased urinary copper and decreased serum ceruloplasmin. Hemochromatosis: this is dueto increase in iron. Iron is important but too much leads to free radical damage. It can deposit in your liver, in your pancrease (diabetes), skin (bronzing), joints (arthritis), and heart (cardiomyopathy). What causes all this iron in the first place? Well it can be from too much blood transfusions if you have things like sickle cell or beta thalassemia. Otherwise, you can have hereditary hemochromatosis, due to mutations in AR C282y or H63D, and is associated with HLA-A3. Important facts you should know is women present later because of their menstrual cycle. Lastly, know that on biopsy iron looks brown, very siilar to lipofuscin cells! This is the wear-and-tear cells and to differentiate them is that iron stains with Prussian blue! Our last topic is on liver cancer! The most common is hepatocellular carcinoma. This is associated with any cause of cirrhosis, as well as things like aflatoxin (toxin found in grain made by Aspergillus. Findings include signs of cirrhosis and quick deterioration. Monitor with AFP and diagnosis with imaging and biopsy. Less common liver tumors include: Cavernous hemangioma: common benign tumor of blood vessels. Don't biopsy as it'll bleed! Hepatic adenoma: associated with contraceptive pill or anabolic steroid. Angiosarcoma: malignant, associated with arsenic and vinyl chloride.

Khun Chirakitti had generally enjoyed good health for his life, but a bad new end happen on 2008. HCC clinical trial data from ESMO 2018

Kate Kelley, MD, of the UCSF Helen Diller Family Comprehensive Cancer Center, San Francisco, CA, shares her thoughts on exciting clinical trial data in hepatocellular carcinoma (HCC) being presented at the European Society for Medical Oncology (ESMO) 2018 Congress, in Munich, Germany. Dr Kelley begins by discussing the benefits of ramucirumab in sub-groups of patients with high alpha-fetoprotein (AFP), from the REACH I (NCT01140347) and REACH II (NCT02435433) studies. These results differed from those seen in the CELESTIAL study (NCT01908426), in which cabozantinib resulted in an improvement on a larger range of AFP positive patients. At this ESMO meeting, the role of checkpoint inhibition in immunotherapy for HCC is also being considered, with data to be presented on SHR-1210 in patients who have previously received sorafenib.
