Living Life Without Prilosec and Other Acid Blockers!

If you are having an ongoing battle with heartburn, acid reflux or GERD (not totally sure of the difference) like I was over the last few years, and wound up taking more Prilosec, Nexium, Prevacid, etc. (acid blockers) than you care to, tune in! Acid blockers are NOT A CURE, they are just a short-term solution (contrary to what doctors may have led you to believe). Hello friends, this is Dr. Rajkumar from Lifeline Hospitals and I am going to be speaking to you today on treatment, especially surgical treatment options for a very common problem faced by millions of people - Acid Reflux Disease - otherwise known as GERD, Gastroesophageal Reflux Disease. In the previous segment we discussed about the cause and today in this segment we will discuss about treatment. To know more about diseases like Gerd, acid reflux, stomach cancer and obesity, we would like you to subscribe to this channel to understand about yourself and about the various treatment options available. Let's discuss today about surgical treatment options for Gerd. Does everyone with a heartburn require to be cartered to the operation table? Nope, we tend to evaluate patients based on how long they have had their acid peptic disease or Gerd and usually people who have suffered for at least two years, who have responded to PPI's or proton pump inhibitors like omeprazole, rabeprazole etc. - responded and then bounced back again with the heartburn and all of these patients are subjected to a 24 hour period study. What is this study? It is a special study wherein we pass some fine acid sensors through a tube with the size of a hair right into the gullet and stomach. These acid sensors collect data, valuable data for us over a 24 hour period so that we are able to understand. One, how much of acid is being secreted. Two, how much of it is coming backwards. Three, whether it is accompanied by bile so that we can have what's called a combined acid and alkaline reflux. Four, whether the patient has significant symptoms at night the nocturnal refluxes or throughout the day etc. Based on this data we calculate and we are able to evaluate which patient requires laparoscopic surgical treatment. Let's talk about how we do this. Four or five small holes and even that nowadays we can do that even through a single hole so called single port surgery. We evaluate a patient's fitness for surgery then a laparoscopic surgical option is performed wherein we tighten up the muscle called the diaphragm through which the gullet passes so that there is a snug fit of the gullet. Then we take the stomach and wrap it around the gullet in such a way that the acid and the alkali are not permitted to come back upwards. The operations sounds complicated but is a fairly simple procedure and it takes me all of 25 or 30 minutes to perform. Patients are usually home in a day or two and over the last 15 years, we must have performed close to 2000 such operations of whom more than 95% are permanently cured of their acid reflux symptoms. A small proportion come back requiring tablets to keep the acid under control. But overall this is an operation that rids the patient's severe heartburn symptoms, prevents the patients from having vomiting by preventing a Barrett's changes on the gullet its actually an operation that will prevent Esophageal cancer, prevents them from having laryngeal cancer and all the other complications that we earlier spoke about. Thus in a well chosen patient with acid reflux disease or Gerd, if he is Evaluated and assessed pre operatively in a proper manner, he can have a fabulous relief of his symptoms and possibly prevent cancer by an excellent laparoscopic or minimally invasive surgical option. A recent study found regular use of proton pump inhibitors (PPIs) for acid reflux raises the risk of stomach cancer. PPI users (Prilosec, Prevacid) in the study had twice the risk for stomach cancer compared to those who used H2-receptor acid reducing drugs (Tagamet, Pepcid). About 20 percent of Americans suffer with acid reflux and heartburn. Most people attribute acid reflux to excess stomach acid. Most often this is not the case. Generally, the problem is too little stomach acid. How does low stomach acid cause acid reflux? The stomach is highly acidic by design so that it can quickly break down foods and kill bacteria, fungi, and other pathogens. It starts the digestion process and acts as a first line of defense for the immune system. Good stomach acidity also helps absorb minerals and signal the rest of the digestive tract to release the right hormones, enzymes, and emulsifiers. Sufficient stomach acid is an important first step in ensuring overall digestion runs smoothly and that you are less susceptible to heartburn, indigestion, belching, gas, food allergies, bacterial infection, and abdominal pain. What causes low stomach acid? Common factors that cause low stomach acid include stress, bacterial infection, poor diet, and nutritional deficiencies. However, an H. pylori infection, which is linked to stomach ulcers, is one of the the most common causes of low stomach acid. If you have any type of pet, this is a common route of transmission. So don’t let them lick your face unless you are absolutely sure they don’t have it and was your hands. With regular soap, not triclosan antibacterial soap. Other factors that play a role in low stomach acid include hypothyroidism, pernicious anemia, and deficiencies in zinc B12, magnesium, or chloride. People who have been vegetarians or vegans for a long time may be deficient in zinc and B12, which are abundant in meats. GERD/Acid Reflux does not get better or even can get worth on Zantac or Prilosec. Why? Zantac belongs to category of H2 blockers and blocks production of HCL in the stomach. its older drug, been on the market for about 20 years and we believe has fewer side effects. Prilosec is in the group of PPIs (proton pump inhibitors )together with Nexium, Prevacid. They block production on H+ by inhibiting H+ pump mechanism, part of HCL (Hydrochloric Acid) production. PPIs considered to be stronger medications. What can I do about acid reflux during pregnancy?

Pregnancy would not be pregnancy without heartburn. You can't find a pregnant woman without antacids in her purse, because it's just a normal part of pregnancy, and it typically goes away after you deliver. Some women just have a mild case of it and others experience more severe symptoms. If you've never experienced heartburn before, it just feels like burning in the chest or in your throat. Sometimes it can make it difficult to swallow, and you may also have a little bit of a raspy voice or a cough to go along with it. If you've noticed any of these symptoms and weren't aware that it's heartburn, talk with your doctor. And after asking you more specific questions about what you're experiencing, they can tell you if it is indeed acid reflux or not. There are some medications that a doctor can prescribe to help with reflux. And there's 4 main types to help reduce the amount of stomach acid that you have that's coming back up your esophagus. And they all work in slightly different ways, and so a woman might try one, and if it doesn't work, her doctor may recommend another type, and then they can continue to determine if other types are warranted in order to help the woman feel more comfortable. There are some other things you can do that don't involve medication at all. For example, if you lay down within 3 hours after eating, you're more likely to have problems with reflux. You can also try sleeping with the head of your bed elevated by either using pillows or actually changing the incline of your bed so that you're slightly raised when you sleep. Tight-fitting clothes can actually increase the chance of having acid reflux as well. So when you're pregnant, everything just feels tight, but if you feel like it might be your clothes that are constricting your stomach a little bit more, see if you can do something about that. That might help with your symptoms. The foods we eat can also contribute to reflux. And so if you're having a problem with it, stay away from chocolate (which is really sad), also colas, coffee, citrus, and fatty foods. They can all contribute to it, and I suggest cutting one out at a time for a few days and seeing if that makes a difference in how you feel. Again, if these remedies aren't helping, or if your doctor has prescribed a medication that doesn't seem to be working, talk with them again, and they'll determine if another type of intervention is warranted.
