It is one of the most common types of cancer in women worldwide, but also one of the most preventable, thanks to early detection with Pap tests. The cervix has 2 major cell types: flat squamous cells lining the outer part, and column-shaped glandular cells covering the inside of the cervical canal. Both types can become cancerous but squamous cell carcinomas are MUCH more common. Cancer usually starts in the zone where the two cell types meet, known as the transformation zone. Virtually all cervical cancers are caused by human papillomaviruses, or HPVs. There are over a hundred different types of HPV, some of which pose HIGHER risks than others. About 70% of all cases are caused by just TWO types: HPV-16 and HPV-18. Two proteins produced by HPV, known as E6 and E7, INTERFERE with cell functions that normally PREVENT EXCESSIVE cell division. This causes the cells to grow in an UNcontrolled manner. HPV is sexually transmitted and is VERY common, but in most women, HPV infections resolve on their own and do NOT cause cancers. Factors that may INcrease the risk of PERSISTENT HPV infections include WEAKENED immune system, other sexually transmitted diseases and smoking. Chances of developing cervical cancer also increase with having many children and LONG-term use of birth control pills. Early-stage cervical cancer generally produces NO symptoms. Advanced-stage disease may cause ABnormal or IRregular vaginal bleeding, pelvic pain, or unusual vaginal discharge. EARLY detection is the key to prevent cervical cancer. Cervical cancer screening may include pap tests ALONE, or in combination with HPV DNA tests. In a pap test, cells are scraped from the cervix and examined for PRE-cancerous changes, known as cervical intraepithelial neoplasia, or cervical DYSPLASIA. These morphological changes can range from mild to severe. If the results are ABnormal, the test is repeated again after 6 months or a year to MONITOR the condition. Additional diagnostic tests may also be performed. In most cases, MILD dysplasia resolves on its own and a follow-up pap test is all that is required to confirm. In a small number of cases, these ABnormal cells may develop into cancer, but they usually take YEARS to do so, which allows plenty of time for treatment when detected early. In the US, a pap test is recommended every 3 years, from the age of 21, or every 5 years if combined with an HPV test. Treatment options for cervical cancer include surgery, radiation, chemotherapy or a combination of these. Early-stage cervical cancer is typically treated with surgical removal of the uterus. This option is the most effective in preventing cancer from coming back and is usually preferred when patients do NOT need to maintain fertility.

Yeah, Paps are a (somewhat literal) pain, but cervical cancer was once the number-one leading cause of cancer death in American, according to the Centers for Disease Control and Prevention. Now, thanks to Paps, which can ID issues before they become cancerous, it doesn't even crack the top 10. Still, the American Cancer Society estimates that 13,240 new cases of invasive cervical cancer will be diagnosed in 2018—31 percent of which will be fatal. And the CDC reports that six out of every 10 cervical cancers occur in women who have never received a Pap test or have not been tested in the past five years. (FYI, all women between the ages of 21 and 65 years old should be getting a Pap smear every three years from 21 to 30, and every five years from 30 to 65, according to the CDC.) “In fact, one of the biggest risk factors of getting cervical cancer is not having had a Pap smear within the past five years," says Eloise Chapman-Davis, M.D., a gynecological oncologist at Weill Cornell Medicine and New York-Presbyterian. However, she notes that you shouldn’t just rely on a Pap to flag any issues. It's also important to watch out for cervical cancer symptoms, which can develop in cases of more developed cancer growths. Translation: If you spot any symptoms of cervical cancer, it's worth talking to your gyno and getting checked out asap. 1. Totally Not-Normal Vaginal Bleeding “One of the most common symptoms of cervical cancer is vaginal bleeding, whether it’s in between your periods, after sex, or after menopause,” says Taraneh Shirazian, M.D., a gynecologist at NYU Langone Health. Abnormal vaginal bleeding is generally a symptom of advanced cervical cancer, because it means that a tumor on the cervix is spreading to affect nearby tissue, Chapman-Davis adds. Periods That Are Heavy AF This isn’t about your period lasting a day longer or suddenly seeming a bit darker. “Rather, it’s your period all of sudden lasting two weeks instead of four days, or having two periods in one month,” says Shirazian. But to play it safe, it you have any changes in your cycle that last for at least two cycles (heavier, lighter, whatever!), it's worth talking to your gynecologist, she says. 3. WTF-Worthy Vaginal Discharge Discharge is totally normal, but the type of discharge you experience could be an indicator of a number of different vaginal health issues. “With cervical cancer, you might notice a discharge that’s foul-smelling and pink, brown, or bloody, potentially with chunks of tissue, or what we call necrotic material,” says Shirazian. And because “masses and tumors secrete fluid, that could contribute to a continuous, watery discharge that seems to occur for no reason,” says Chapman-Davis. Go ahead and give your gyno a call. 4. Pelvic, Back, or Leg Pain Pelvic pain could be an indicator of changes to the cervix, but advanced cervical cancer can even spread to the bladder, intestines, or even the lungs and liver, says Chapman-Davis. “Then you might have things like back pain or leg pain,” she says. “But that’s typically associated with very advanced cases because the cervix isn’t really affecting a lot of nerves.” Talk to your primary care doc to rule out cervical cancer as well as other potential nerve causes. 5. Major Fatigue Because most cervical cancer symptoms don’t come along until it enters more advanced stages, it does share some symptoms with all cancers. ABNORMAL VAGNIAL BLEEDING Symptoms, Causes and Treatments OVERVIEW Abnormal vaginal bleeding is any vaginal bleeding unrelated to normal menstruation. This type of bleeding may include spotting of small amounts of blood between periods — often seen on toilet tissue after wiping — or extremely heavy periods in which you soak a pad or tampon every one to two hours for two or more hours. Normal vaginal bleeding, or menstruation, occurs every 21 to 35 days when the uterus sheds its lining, marking the start of a new reproductive cycle. A menstrual period may last for just a few days or up to a week. Your flow may be heavy or light and still considered normal. Menstrual cycles tend to be longer for teens and for women nearing menopause, and menstrual flow may also be heavier at those ages. CAUSES Abnormal vaginal bleeding can relate to an issue with your reproductive system (a gynecologic condition) or to other medical problems or certain medications. If you have reached menopause — defined as 12 consecutive months, give or take, without a menstrual period — subsequent vaginal bleeding may be a cause for concern and should be evaluated. Possible causes of abnormal vaginal bleeding include: • Cancers and precancerous conditions • Cervical cancer • Endometrial cancer (uterine cancer) • Endometrial hyperplasia • Ovarian cancer • Uterine sarcoma • Vaginal cancer TREATMENT This will depend on the cause of the abnormal uterine bleeding -- if a chronic illness or a blood disorder is at the root of your symptoms, treating that can help. Your treatment also can depend on whether you plan to have children. It might not be safe to get pregnant after some treatments, while others can make it impossible. If you’re close to menopause, your doctor may want to take a wait-and-see approach because your symptoms may get better on their own. Drugs are usually the first thing your doctor will try: Hormones. Birth control pills and other hormone treatments may be able to give you regular menstrual cycles and lighter periods. Gonadotropin-releasing hormone agonists (GnRHa). These stop your body from making certain hormones. They can shrink fibroids for a while, but they’re usually used along with other treatments. NSAIDS. If you take anti-inflammatories like ibuprofen or naproxen a few days before your period starts, they may help lighten the bleeding. Tranexamic acid. This is a pill that helps your blood clot and can control heavy uterine bleeding. Sometimes surgery can be needed to stop the bleeding: Endometrial ablation. This uses heat, cold, electricity, or a laser to destroy the lining of the uterus. It may end your periods entirely. You probably won’t be able to get pregnant after having it done, but it can be very dangerous if you do. You’ll need to use birth control until menopause. Myomectomy or uterine artery embolization. If you have fibroids, your doctor may take them out or cut off the vessels that supply them with blood. Hysterectomy. This is when your uterus is taken out. You may need a hysterectomy if your fibroids are very large or you have endometrial or uterine cancer. Otherwise, it’s a last resort when other treatments haven’t worked.

Still, the American Cancer Society estimates that 13,240 new cases of invasive cervical cancer will be diagnosed in 2018—31 percent of which will be fatal. And the CDC reports that six out of every 10 cervical cancers occur in women who have never received a Pap test or have not been tested in the past five years. (FYI, all women between the ages of 21 and 65 years old should be getting a Pap smear every three years from 21 to 30, and every five years from 30 to 65, according to the CDC.) “In fact, one of the biggest risk factors of getting cervical cancer is not having had a Pap smear within the past five years," says Eloise Chapman-Davis, M.D., a gynecological oncologist at Weill Cornell Medicine and New York-Presbyterian. However, she notes that you shouldn’t just rely on a Pap to flag any issues. It's also important to watch out for cervical cancer symptoms, which can develop in cases of more developed cancer growths. Totally Not-Normal Vaginal Bleeding “One of the most common symptoms of cervical cancer is vaginal bleeding, whether it’s in between your periods, after sex, or after menopause,” says Taraneh Shirazian, M.D., a gynecologist at NYU Langone Health. That surgery tends to be very successful, he says. On the other hand, if you're noticing symptoms, your cancer may be fairly advanced. "A cure is still possible at that stage, but it's a lot more complex," Anderson says. That cure will probably involve radiation or chemotherapy, and the outcomes are less certain, he says. 1.Abnormal bleeding Unusual vaginal bleeding is the symptom most strongly linked to cervical cancer, says Joshua Cohen, MD, a gynecologic oncologist at UCLA. "This could be bleeding after intercourse or between periods," he says. Anderson says all of the above symptoms—if caused by cervical cancer—are most likely to appear during your late 30s, 40s, or 50s. "We see cervical cancer in women as young as 20, and also much later in life, but the median age is mid-40s," he explains. Matt McDonald, MD: A common question I am asked is what are the warning signs and symptoms of cervical cancer? And when thinking of cervical cancer, it's important to categorize that as precancer or precursor of a cancer called cervical dysplasia and cervical cancer. Cervical cancer in and of itself really the most common warning sign or symptom from that really is abnormal bleeding. Most importantly or what you'll read in a textbook is something called postcoital bleeding which is unforeseen or abnormal vaginal bleeding after sexual intercourse and that usually happens because the cervical cancerous cells are irritated during intercourse and they are friable and bleed easily when they are touched. So if a woman complains of abnormal bleeding after intercourse, they certainly should seek attention by their physician. Other warning signs that we see with cervical cancer are more indolent and insidious and what I mean by that is they really don't have any symptoms related to the GYN organs. At lot of times when we see patients come in with advanced cervical cancer, which is when we really don't want to catch patients, we have symptoms that really have to do with the kidneys not working or otherwise known as kidney failure that happens sometimes when the cervical cancer becomes so large that it will block the flow of urine from the kidney to the bladder and result in kidney failure and those patients come in with all kinds of symptoms of fatigue, nausea, etc., that really are symptoms related to the kidney failure not necessarily to the cervix. So our goal really is to catch women and find cervical cancer in its early stages. In the early stage symptoms usually have to do with this entity of postcoital bleeding. Alarmingly, though a lot of patients when they develop early cervical cancer like stage 1 and stage 2 have no symptoms not even the abnormal bleeding and so what we really try to do is catch cervical cancer before it exist and that is where we go from a screening standpoint in hopes of preventing cervical cancer and the most effective means of preventing cervical cancer is the Pap smear, and that requires regular visits to the gynecologist or your family practitioner, etc. That's the most effective means of preventing cervical cancer. Without Pap smears, we have no idea where our patent is in the continuum of cervical dysplasia and that's why in certain developing countries where Pap smear surveillance is not a regular part of a woman's life, cervical cancers still the leading cause of cancer related deaths in some women. In countries such as the United States, where cervical cancer screening with Pap smears is a regular event, we have done an enormous job at preventing cervical cancers. Abnormal uterine bleeding accounts for 1/3 of all visits to a gynecologist's office. And most women come in with concerns about having too much or too little bleeding, having periods that are abnormally short or too long, having irregular cycles, or maybe none at all. You should definitely call your doctor if you have periods lasting longer than 8 days or you have abnormally heavy periods as evidenced by the fact that you're passing clots, having to wear both pads and tampons because it's so heavy, or you have to change one or the other every hour or 2, or if you have to get up in the night and change them. That's not normal, and that means that you're having abnormally heavy periods. Also call the doctor if you've gone a total of 3 months without a period and pregnancy has been ruled out as the cause, if you have bleeding or spotting between periods or after intercourse, if you have such painful periods that it affects your ability to function each month - you feel like everything just has to shut down, life has to stop for you - or if you notice a change in the color, odor, or consistency of vaginal discharge, and if you notice a fever associated with abnormal uterine bleeding patterns. All of these things should be reported to your doctor. They'll ask you more specific questions about your observations and concerns, performa an exam, and then decide if further investigation or treatment is necessary for you. Some of the most common causes that they'll look into are fibroids, polyps, adenomyosis, (of course) pregnancy should also be ruled out, cancer can be a cause (although it's rare), sometimes infection contributes to it, and birth control can also cause it. If you've been on certain types of birth control, they'll talk to you about whether or not the symptoms you've been having are normally associated with those types of birth control, or if it's abnormal. And if that's the case, then maybe another type of birth control would be better for you. The doctor will also consider other underlying medical conditions that can contribute to it - things like clotting disorders, hyperthyroidism, kidney renal disease, and endocrine problems. One other thing the doctor will consider is the fact that maybe the bleeding isn't coming from the vagina at all. Sometimes a woman has bleeding from the urinary tract, or from the rectum, and they can determine where it's coming from just by performing a simple exam. Now the treatment for abnormal uterine bleeding depends on the cause, and sometimes it means medication, sometimes it means surgery, and another thing a doctor will consider (if you've been having a lot of bleeding or heavy bleeding) is anemia, or basically, you lost so much blood that it leaves you feeling very fatigued, you have lack of energy, you might feel a little bit dizzy.
