WOMAN 3: A silent wall of shame grew between us IGOR VOLSKY: These women suffer from lack of sexual desire and now a new pill developed by Sprout Pharmaceuticals could help jump start their libido. JAMES SIMON: This has been under the radar, hidden, most women are just sucking it up, so to speak. IGOR VOLSKY: After years with no FDA-approved solutions, Flibanserin -- popularly known as “the female Viagra” -- may finally provide women with treatment. In fact, women’s health groups have joined with the pharmaceutical company to create a new campaign called Even The Score. That campaign is lobbying the FDA to approve Flibanserin. It's about time, right? CINDY PEARSON: Even The Score is full of lies, knowing willful lies. IGOR VOLSKY: One such lie, the campaign strongly implies the drug is similar to Viagra, but that's not true at all. Men take the little blue pill when they want to have sex to increase blood flow to the penis. Women would have to take Flibanserin daily -- and that pill works to convince the brain to desire sex. Also, as one of the doctors advising the FDA told us, this drug isn’t for nearly any women. CALEB ALEXANDER: In general medicine, in my clinical practice of 15 years, I’ve never met a woman that fulfills the formal diagnostic criteria for this disorder. IGOR VOLSKY: And for the few pre-menopausal women who do qualify, they would have to make some major life changes. The only way I could take this product as a woman is: I have to stop drinking, I have to get off my prescription birth control, contraception pill... CALEB ALEXANDER: No anti-fungal medicine for vaginal yeast infections. IGOR VOLSKY: Ok, stop that. And I have to take it at night and over a six month period I might, during some point in that six months experience a slightly heightened desire to have sex. CALEB ALEXANDER: Right. IGOR VOLSKY: In fact, the few participants who made such a small increase in their sexual activity that we had trouble characterizing it. And many participants experienced adverse effects that the expert panelists feared would only increase in the general population. CALEB ALEXANDER: If you faint while you drive a car, you may kill yourself or someone else. These safety concerns in light of the very modest effectiveness benefit together don’t present a favorable package. IGOR VOLSKY: The FDA previously rejected Flibanserin twice and this third time around, they requested an additional study of how the drug interacts with alcohol. The company enrolled 25 people: 23 men and only 2 women -- and later claimed it didn’t have the ‘foresight’ to enroll more women. CINDY PEARSON: Sprout Pharmaceuticals set off to do the right thing, I believe, but they then went astray. When the FDA said, we’re still not convinced, you still haven’t answered all the questions about safety, then they started advancing pretty significant amounts of money into this marketing campaign IGOR VOLSKY: After a full day of hearings, the panel voted 18-6 to approve the drug, though not without conditions and some serious reservations. PANELIST 1: Women suffering from HSDD deserve better than this. PANELIST 2: I think we all wish it was a drug that was a better one. PANELIST 3: There are serious safety concerns, so my yes is conditional. PANELIST 4: If this were the 7th drug in the class, I think it would be a very different discussion. IGOR VOLSKY: So will this drug really help women? JAMES SIMON: It is not perfect. The drug is not perfect, the FDA is not perfect. We accept certain imperfections to move progress. CINDY PEARSON: If the FDA approves flibanserin, I will be convinced forever that it came about as a result of a brilliant marketing campaign and not the result of good science and I’m afraid women will suffer as a result.

Having several women friends who had confidentially said the opposite, I realize this might make a terrific video parody. So I get up and start writing knowing the most important visual catchphrase, a woman in bed with her knees bent, would be my opening. But who would I cast as the actress? I had hired a woman (Delores Bernardi) to do my filing several months earlier. The first comment I had made as she walked into my loft was: "Wow, what a commercial face you have." Plus, her voice reminded me of the wonderful Brenda Vaccaro's. So I talked Dee into doing the video despite her many fears that she would never remember lines, hated being in front of a camera, had no showbiz aspirations. My instincts were correct. She did a fantastic job. The reason for creating "Noodling 'Round With Viagra" is to gain attention, as a writer, to enable me to sell my film scripts. As a former professional photographer, magazine writer, and now script writer, my soon to be 78 years is an obstacle. Of course I create characters who are older such as in my comedy Teach Me Tonight, a naughty comedy about two women, in their early 50s, who hire a faux gay man, to teach them what they've long forgotten about sex. Specifically, flibanserin increases dopamine and norepinephrine, responsible for sexual excitement, and decreases serotonin, which is responsible for sexual satiety and inhibition. This is likely achieved by a reduction of glutamate transmission. Women who took the drug in a clinical study have shown an increase of about 1.0 more sexually satisfying events per month, Reuters reported. The pill needs to be taken every day and its most common side effects are dizziness, nausea and sleepiness. I am asked frequently if women can take Viagra. Men and women generally have different issues. Men usually complain of performance problems, not sex drive. Women "usually" are looking to improve their low libido. If they have performance issues, it is more related to the vaginal changes of menopause. Studies in women were very disappointing, because it does NOT improve libido, and should have been obvious at the onset of the study. A new medicine, “Flibanserin" is a novel drug, that reportedly works to improve libido in women by stimulating the brain. It was denied by the FDA, but has just resubmitted an application to be approved. It stimulates neurotransmitters in the brain, similar to an antidepressant, but it does it in a different part of the brain.

Urdu/Eng/Nat Doctors in Pakistan are warning of the dangers of men buying the impotence pill Viagra on the black market and taking it without prescription. The sale of the drug in the country is currently being debated by the government - it says it wants to make sure the drug will be made available in the safest possible way. But medical experts say the problem of impotency is high in the country and that men are anxious to try out supplies of Viagra introduced illegally at a high price. Viagra, the impotency pill, is proving to be big business in Pakistan. The drug, first released in the U-S in March 1998, is legally available in more than 50 countries worldwide. Many people consider it the beginning of a new revolution in pharmaceuticals. In Pakistan, the government is currently discussing when and how to introduce the drug. But Viagra has already found its way onto the black market in the country's capital Islamabad and can be bought at a high price. One tablet can cost anything from 20 to 50 U-S dollars - a bottle of Viagra tablets can cost anything from 360 to 800 U-S dollars. Some Viagra users say the drug has been freely available to them for some time now and that they're prepared to pay up. SOUNDBITE: (Urdu) "Viagra is a sex drug and I have been using it for about two years and found it very good, because it adds energy to the body if we use it two hours before sexual intercourse. I have found no other medicine better than Viagra and the demand for it is now growing." SUPER CAPTION: Abdul Qadir, Viagra user The country's health department has so far been reluctant to legalise Viagra - but it is aware of the black market problem and is prosecuting anyone found selling the drug. Government officials say they want to make sure that there are no loopholes before Viagra is legally available in Pakistan. SOUNDBITE: (English) "In the West they know that this drug has got side effects and that it is a prescription drug. So nobody can buy it without prescription, while in Pakistan we still have some loopholes, some weaknesses where the drug can be sold over the counter and this is my main worry in resisting to register it." But Pfizer, the company that produces Viagra, maintains the drug is a safe medicine for erectile dysfunction when used as recommended in the approved package insert. There are already many legal drugs on sale in Pakistan to help enhance sex. Herbal doctors offer remedies for impotency, claiming they're natural products that have no side effects. The herbal remedy sellers can often be found on roadsides doing a roaring trade - but even they now say that the new wonder drug is threatening their trade. Medical experts say there is no official impotency figure in the country because Pakistani men are reluctant to admit they have a problem in a nation where sex is a taboo subject. But doctors say it is an ever increasing problem and they are warning of the dangers of taking the impotence pill without prescription. Doctor Khalid Saeed said it is even more of a worry as impotency affects men of all ages. SOUNDBITE: (English) "There are two basic main groups in which we can find this who come to us commonly. One is the younger age group - they are really people in their early 20s and the other group is somewhere in the late 40s and 50s and both of them have different presentations. People in the younger age group generally come to you just before they are about to get married." Tagalog/Eng/Nat Viagra, the headline grabbing anti-impotency pill, is now on sale in the Philippines. Pfizer, who manufacture the drug, have started delivering to local drugstores in Manila, two weeks after the local Bureau of Food and Drug gave its approval for its sale in the country. Pharmacies in the Philippines have finally begun to receive their stocks of Viagra. The manufacturer Pfizer, started deliveries of the new wonder drug at drugstores in Manila this week. Some stores already began selling the drug on Tuesday. With Viagra becoming the talk of the town, drugstore owners are anticipating a huge demand. Pharmacy owner Bhoyette Vizconde sold five tablets on the first day. SOUNDBITE: (English) "I already hoard (laughs) stocks because I'm really expecting that being the first to have Viagra here in our vicinity, I'm sure a lot of people will come here and inquire or buy Viagra." SUPER CAPTION: Bhoyette Vizconde, Owner of Marcelo's Pharmacy Pharmacy Plus received its first supply early this week. Owner James Sandoval, who owns several stores in the capital, put a few boxes of the pill on the shelf to show clients that the drug is now available. But before the drug was put on sale in his stores, Sandoval issued a memo to all his store managers to ensure that safety precautions are practiced before the drug is dispensed to patients. The B-F-A-D says it is important the pill is only sold to patients who come with a legitimate prescription from their doctors. Each pill comes with a leaflet giving warnings of possible side-effects if the drug is not taken properly. Pfizer warned that the drug should not be taken by people being treated with nitrate-based medication for heart problems. While he expects a huge demand for the drug, Sandoval said Filipino men may be a little shy of asking for the drug themselves. He says the men here will probably not want to admit they need Viagra and so he is arranging a home delivery service to put them out of their misery. SOUNDBITE: (English) "Because maybe at the secrecy of their own home, they can acquire the product because they might be too embarrassed to come forward to admit that they have a sexual problem. Any person that comes forward asking about the product - they're so defensive, they say right away "Hey listen I don't have a problem with this. I'm just inquiring". That goes to tell you how sensitive the issue is." SUPER CAPTION: James Sandoval, Owner of A-B Sandoval Drugstore Dr. Jesus Mendoza is a local doctor who has been checking out the testing of the Viagra drug. SOUNDBITE: (English) "Well, I was pretty excited and wanted to find out whether it was really going to work. Of course, there was some degree of skepticism at first. But when we started seeing back the patients who were on the study, then it was confirmed that my gosh, this works." SUPER CAPTION: Dr. Jesus Mendoza, Urologist In a press conference early this month, Pfizer said the approval from B-F-A-D came after they completed a six-month study of its effects on 500 men from six Asian countries, including the Philippines. The man who discovered Viagra, Dr. Peter Ellis, was in Manila last week and spoke of how his drug differs from the others already available in the market. SOUNDBITE: (English) "And I think the beauty of the drug is that it works to enhance the natural process. All the other products that are out there, you take the product, you get an erection. But Viagra, you take the product, you don't get an erection but it's only when sexually aroused. So it's basically enhancing a perfectly natural function and restoring it back to normal." US Food and Drug Administration has finally given a green signal to the pill, which they had twice rejected due to insignificant benefits and worrisome side effects.
