Breast cancer patients who are not candidates for, or choose not to undergo nipple-sparing mastectomy also have the option to reconstruct their nipple and areola as part of their breast reconstruction. Although there are many different ways to perform nipple reconstruction, at PRMA we use the "bow-tie" method. The procedure gets it's name from the bow-tie shaped incision used to create the new nipple. The PRMA surgeons perform nipple reconstruction by cutting a "bow-tie" shaped skin flap on the breast. The flap is lifted, shaped, and sutured to create a "natural", projecting nipple. The procedure can be done during revision surgery in the operating room, or as a separate procedure in the office under local anesthetic. Fat grafting is a fairly new technique in breast surgery. Fat is liposuctioned from one part of the patient's body, purified and then injected into the breast. There are several fat grafting techniques that are used by plastic surgeons. There is no "set way" that has been shown to be the best in terms of long-term results. Incredible video here of a bipedicled conjoined DIEP flap! During bilateral DIEP flap breast reconstruction, the lower abdominal tissue shown here would normally be split down the middle creating two “flaps”, one flap for each breast. However, for this unilateral (one-sided) breast reconstruction, we will leave the two flaps connected in the middle (“conjoined”) and transfer it to the chest as one large piece of tissue. Since we need to use all the lower abdominal tissue to match the remaining breast in this patient, we need to include blood vessels from both sides of the abdomen. This ensures all the tissue has a good blood supply and survives fully after it is transplanted to the chest to create the new breast. Both sets of blood vessels are reconnected to the blood vessels in the chest using microsurgery. This is a great option for thinner patients who desire breast reconstruction and need more than one flap to match the size of the other breast. Patients who would otherwise not be considered candidates for autologous tissue (your own tissue), are routinely given this option at PRMA.
